Abatacept in IgG4-related disease: a prospective, open-label, single-arm, single-centre, proof-of-concept study

Summary: This study investigated the efficacy and safety of abatacept in the treatment of IgG4-related disease. After 24 weeks of treatment, 30% of patients achieved complete remission. Higher baseline proportions of unswitched memory B cells were associated with the efficacy of abatacept. Reductions in serum IgE concentrations, circulating plasmablasts, and activated type 2 T follicular helper cells were also related to treatment response.

Most patients had involvement of multiple organs, presented with nonspecific initial clinical manifestations, and often had mediastinal/hilar lymph node involvement.

Summary: Background Immunoglobulin G4-related disease (IgG4-RD) is a newly recognized systemic, immune-mediated, and fibro-inflammatory disease. Low complement levels were found in 20.8% of untreated IgG4-RD patients. Patients with hypocomplementemia had more involved organs, higher IgG4-RD responder index, and higher levels of various laboratory parameters compared to those with normal complement levels.
