Inter-fraction dynamics during post-operative 5 fraction cavity hypofractionated stereotactic radiotherapy with a MR LINAC: a prospective serial imaging study

Hendrick Tan et al.

Summary: This study investigated changes in clinical target volume (CTV) during hypofractionated stereotactic radiotherapy for resected brain metastases, showing significant reductions in CTV during treatment. The results support the use of MR image guided radiation therapy and treatment adaptation to mitigate toxicity, while also highlighting significant CTV contouring variability between different imaging sequences.

Single- and Multifraction Stereotactic Radiosurgery Dose/Volume Tolerances of the Brain

Michael T. Milano et al.

Summary: This study pooled data from 51 reports to investigate radiation-induced brain toxicity after hypofractionated radiation therapy. Results showed wide variability in rates of radionecrosis, with single-fraction SRS tissue volumes receiving 12 Gy associated with risks of symptomatic radionecrosis. The use of fSRS appears to reduce risks of radionecrosis for larger treatment volumes compared to SRS.

Summary: The study compared the incidence of treatment-related necrosis in patients with brain metastases receiving combination SRS+ICI therapy versus SRS therapy alone. Results showed a significantly higher incidence of necrosis in the combination therapy group.

The combined use of steroids and immune checkpoint inhibitors in brain metastasis patients: a systematic review and meta-analysis

Charissa A. C. Jessurun et al.

Summary: A systematic review and meta-analysis of 15 observational studies revealed that the use of steroids in BM patients receiving ICI was associated with significantly worse overall survival and systemic progression-free survival. Further research on the dosage, timing, and duration of steroids is needed to optimize outcomes in these patients.

Current status and recent advances in resection cavity irradiation of brain metastases

Giuseppe Minniti et al.

Summary: This review discusses the use of postoperative stereotactic radiosurgery (SRS) as an effective treatment option for resected brain metastases, providing superior local control compared to surgery alone and decreasing the risk of neurocognitive decline compared to whole brain radiotherapy (WBRT). Further investigation is needed to optimize dose prescription/fractionation, timing of treatment, and target delineation for improved outcomes.
