The authors reported a case of a child with chronic recurrent multifocal osteomyelitis (CRMO) and Crohn disease (CD) who responded successfully to Crohn disease exclusion diet (CDED) in combination therapy, indicating the potential benefit of diet in resistant patients with CD and extraintestinal manifestations.

Summary: Chronic nonbacterial osteomyelitis (CNO) is a complex autoinflammatory disease with diverse phenotypes and associations with other autoimmune conditions. This study in Ireland aimed to provide a detailed description of children and adolescents with CNO, showing a high prevalence of extrabone manifestations and a low response rate to NSAID treatment. The need for defining different subtypes of CNO is highlighted by the findings of this national cohort study.

Paradoxical Psoriasis in Children Receiving Anti-TNFα Treatment for Inflammatory/autoimmune Disease

Summary: TNFi are commonly used in children with autoimmune and autoinflammatory conditions, but can paradoxically induce psoriasiform lesions. Paradoxical psoriasis may have a higher risk in patients with JIA and IBD after TNFi use, with incompletely defined risk factors. Topical therapies are often effective in treating psoriasis, but severe cases may require TNFi switching or discontinuation. Further research is needed to better characterize risk factors and understand disease pathogenesis.

Pediatric Patients with a Dual Diagnosis of Inflammatory Bowel Disease and Chronic Recurrent Multifocal Osteomyelitis

Summary: This study conducted a retrospective chart review of patients with a dual diagnosis of IBD and CRMO, identifying seven patients with this dual diagnosis. Most patients were initially diagnosed with IBD before being diagnosed with CRMO, and treatment for IBD was continued during the CRMO diagnosis. The study showed that the patient population responded well to biologic agents, particularly tumor necrosis factor-alpha inhibitors, for both conditions.
