A randomized, double-blind, placebo-controlled trial of hydrocortisone augmentation of Prolonged Exposure for PTSD in US combat veterans

Summary: Cognitive behavioral therapies like Prolonged Exposure (PE) are commonly used to treat PTSD, but adding the synthetic glucocorticoid hydrocortisone (HCORT) to the treatment did not significantly improve symptoms overall. However, exploratory analyses showed that veterans with mild TBI exposure and current postconcussive symptoms experienced a greater reduction in hyperarousal symptoms with HCORT augmentation during PE treatment. Additionally, veterans with higher baseline glucocorticoid sensitivity showed a greater reduction in avoidance symptoms with HCORT augmentation.

Summary: This study conducted a meta-analysis on the potential intervention of Hydrocortisone in the treatment of PTSD, finding a significant effect in prevention but weaker effect in treatment. Therefore, Hydrocortisone may be a promising and cost-effective medication for preventing PTSD, but further rigorous studies are needed to confirm its efficacy.
