General Anesthesia Versus Conscious Sedation in Endovascular Thrombectomy for Stroke: A Meta-analysis of 4 Randomized Controlled Trials

Doug Campbell et al.

Summary: Randomized controlled trials comparing general anesthesia (GA) to conscious sedation (CS) in endovascular thrombectomy patients showed that GA patients achieved higher successful recanalization and good functional outcomes, with no significant differences in intracerebral hemorrhage and 3-month mortality compared to CS patients.

Thrombectomy for Primary Distal Posterior Cerebral Artery Occlusion Stroke The TOPMOST Study

Lukas Meyer et al.

Summary: This study suggests that mechanical thrombectomy is a safe and technically feasible treatment option for patients with posterior circulation distal, medium vessel occlusion, compared to standard medical treatment with or without IVT.

Endovascular Therapy for Stroke Due to Basilar-Artery Occlusion

L. C. M. Langezaal et al.

Summary: In a randomized trial involving 300 patients with basilar-artery stroke, endovascular thrombectomy was not significantly different from medical therapy with respect to a favorable functional outcome (modified Rankin scale score of 0 to 3) at 90 days, but a clinically significant benefit could not be excluded.

Hemodynamic Status During Endovascular Stroke Treatment: Association of Blood Pressure with Functional Outcome

Min Chen et al.

Summary: This study found that intraprocedural pre-recanalization blood pressure reductions during endovascular therapy were not associated with functional outcome, but higher pre-recanalization blood pressure was correlated with worse functional outcomes. Prospective randomized controlled studies are needed to determine the feasibility of blood pressure as a treatment target for modifying outcomes.
