Why won't it stop? The dynamics of benzodiazepine resistance in status epilepticus

Summary: Many episodes of status epilepticus do not respond to first-line treatment with benzodiazepines. Seizure-induced alterations to the sensitivity of the GABA receptor to benzodiazepines are presented as a possible mechanism of treatment resistance. Longer episodes of status epilepticus can alter brain physiology, contributing to benzodiazepine resistance. Current management guidelines often do not account for these duration-related changes, which may differentially impact individuals in low-income and middle-income countries.
