Outcomes Associated With Cardiac Arrest in Patients in the Cardiac Intensive Care Unit With Cardiogenic Shock

Abdelrahman M. Ahmed et al.

Summary: This study aimed to determine the prevalence, patient characteristics, and outcomes of cardiac arrest (CA) in patients with cardiogenic shock (CS) admitted to the cardiovascular intensive care unit. The results showed a decreasing prevalence of CA over time and a higher risk of hospital and 1-year mortality associated with CA.

Effect of Intra-arrest Transport, Extracorporeal Cardiopulmonary Resuscitation, and Immediate Invasive Assessment and Treatment on Functional Neurologic Outcome in Refractory Out-of-Hospital Cardiac Arrest A Randomized Clinical Trial

Jan Belohlavek et al.

Summary: The study aimed to determine whether an early invasive approach improves neurologically favorable survival in patients with refractory out-of-hospital cardiac arrest. The results showed no significant difference in survival with neurologically favorable outcome at 180 days between the invasive strategy group and the standard strategy group. However, the invasive strategy group had higher rates of neurologic and cardiac recovery at 30 days.

Summary: A randomized clinical trial was conducted to investigate whether early use of moderate hypothermia during venoarterial ECMO treatment improves mortality in patients with cardiogenic shock. The results showed that early application of moderate hypothermia for 24 hours did not significantly increase survival compared with normothermia.

Dhruv Sarma et al.

Summary: This study investigated the association between shock severity, as defined by the SCAI Shock Classification, and in-hospital mortality and neurological outcome in comatose OHCA patients undergoing TTM. The study found that higher shock severity was associated with increased in-hospital mortality and a lower likelihood of good neurological outcome.

Nilesh Pareek et al.

Summary: Increasing SCAI shock grade after Out of Hospital Cardiac Arrest is associated with 30-day mortality, requirement for renal replacement therapy and mortality attributed to multiorgan dysfunction syndrome and cardiac etiology death. Early coronary angiography is significantly associated with reduced mortality only in SCAI grade D patients.

Vasopressor and Inotrope Therapy in Cardiac Critical Care

Jacob C. Jentzer et al.

Summary: Patients in the cardiac intensive care unit (CICU) often require hemodynamic support with vasopressors and inotropes to restore tissue perfusion. Timely initiation of optimal therapy is essential to achieve effective tissue perfusion and normalize cellular metabolism. Consideration should be given to both arterial pressure and tissue perfusion when choosing therapeutic interventions for patients with shock.

MCS-assisted PCI identified a sicker AMI-CS cohort, with a decrease in IABP and an increase in pLVAD/ECMO from 2009 to 2014. MCS use was independently predicted by younger age, male sex, lower comorbidity, and cardiac arrest, and was predictive of higher in-hospital mortality and greater resource utilisation.
