Summary: This study analyzed the occurrence of de-novo NAFLD at 5-year post-liver transplantation and identified predictive factors including male sex, obesity, metabolic syndrome, and de-novo diabetes. It was also found that a significant proportion of patients had advanced fibrosis, and patients with NAFLD had higher rates of cardiovascular comorbidities.

Summary: Allograft steatosis is a common issue post liver transplantation, with 70% of patients experiencing it according to TE and CAP assessments. Major risk factors for severe steatosis include older age at LT, post-LT obesity, and alcoholic liver disease. TE should be considered as a screening tool for detecting allograft steatosis and fibrosis even in patients with normal liver enzymes.

Although there was no difference in overall survival after 5 years, closer monitoring and aggressive management of cardiovascular risk factors are needed for NAFLD patients post-transplantation.
