Perinatal outcomes in women living with HIV-1 and receiving antiretroviral therapy-a systematic review and meta-analysis

Shiri Shinar et al.

Summary: This systematic review and meta-analysis found that pregnant women living with HIV on antiretroviral therapy have an increased risk of preterm birth, low birthweight, and small-for-gestational age in high resource countries compared with HIV-negative controls. The risk of preterm birth and low birthweight is also higher in low income countries for these women.

The assessment of blood pressure in pregnant women: pitfalls and novel approaches

Alice Hurrell et al.

Summary: Accurate assessment of blood pressure is crucial for safe obstetrical care. Various techniques, including auscultatory method and automated oscillometric devices, are used for blood pressure assessment. Factors such as position, cuff size, conversation, caffeine, smoking, and irregular heart rate can affect the accuracy of blood pressure measurements. There are differences in the classification and treatment thresholds for hypertensive disorders. Novel technologies may aid in early recognition of abnormal blood pressure. The importance of accurate blood pressure assessment is emphasized for predicting preeclampsia and managing severe hypertension.

Prediction and prevention of preterm birth in pregnant women living with HIV on antiretroviral therapy

Amanda J. Jones et al.

Summary: This review discusses the available evidence on the prediction and prevention of preterm birth in pregnant women living with HIV. Further research on the effect of antiretroviral therapy (ART) and the development of predictive tools are crucial in this population. Vaginal progesterone supplementation may be a therapeutic option to prevent recurrent preterm birth in pregnant women living with HIV.

Maternal and infant renal safety following tenofovir disoproxil fumarate exposure during pregnancy in a randomized control trial

Kristin Baltrusaitis et al.

Summary: Tenofovir disoproxil fumarate (TDF) combined with other antiretroviral drugs is commonly used for HIV treatment, but its renal safety during pregnancy is still not well understood. This study evaluated the renal function and calcium-phosphate levels in pregnant women receiving TDF, and found no significant safety concerns for maternal or infant renal function.

Optimizing Pharmacology Studies in Pregnant and Lactating Women Using Lessons From HIV: A Consensus Statement

Ahizechukwu C. Eke et al.

Summary: The workshop discussed the impact of drug exposure during pregnancy and lactation on mothers and infants, addressing the gaps in knowledge on drug selection and dosing in these situations. Collaborative efforts were made to define key standards for generating pharmacology data for antiretrovirals in pregnant and lactating women, with a focus on conducting optimal studies for rapid implementation in clinical settings.

Summary: This study found no significant difference in renal function between pregnant women living with HIV receiving TDF versus TAF. Both TDF and TAF appear to be safe in terms of renal toxicity for pregnant women living with HIV, but larger prospective cohort studies are needed for further confirmation.

High Stillbirth Rate among Human Immunodeficiency Virus-Infected Pregnant Women in West Bengal, India: a Retrospective Cohort Study

Suman Ganguly et al.

Summary: This study compared the stillbirth rate in HIV-infected pregnant women with that in the general population and found a significantly higher stillbirth rate in HIV-infected pregnancies, which was significantly reduced after the initiation of antiretroviral treatment. Spouse testing for HIV and maternal literacy were also found to be significantly effective in preventing stillbirth.

Ethical considerations for research involving pregnant women living with HIV and their young children: a systematic review of the empiric literature and discussion

Catherine G. Raciti et al.

Summary: Ethical considerations regarding research participation of PWLHIV and their young children are crucial, but empirical literature on this topic is limited. Studies identified key themes including adequacy of informed consent, paternal involvement, balancing risks, and access to research and treatment. More research is needed to establish a comprehensive ethical framework for researchers working with PWLHIV.

Adverse Pregnancy Outcomes Among HIV-positive Women in the Era of Universal Antiretroviral Therapy Remain Elevated Compared With HIV-negative Women

Vincent J. Tukei et al.

Summary: The study found that despite receiving antiretroviral therapy, HIV-positive pregnant women were still more likely to experience adverse pregnancy outcomes compared to HIV-negative pregnant women, including intrauterine death and low birth weight. This suggests that relying solely on ART may not fully reduce the negative impact of HIV infection on pregnant women.

Karina W. Davidson et al.

Summary: Gestational diabetes is a common condition during pregnancy in the US, with significant risks for both mothers and infants. The USPSTF recommends screening for gestational diabetes at 24 weeks of gestation or after to improve outcomes for both mother and fetus, but finds insufficient evidence to recommend screening before 24 weeks.

Maternal weight and birth outcomes among women on antiretroviral treatment from conception in a birth surveillance study in Botswana

Rebecca Zash et al.

Summary: This study examined the impact of ART-associated weight gain on pregnancy outcomes among women living with HIV. Low baseline weight was associated with increased risk of very preterm delivery and very small for gestational age, while high baseline weight was associated with increased risk of macrosomia and maternal hypertension. Second trimester weight gain showed weaker associations with outcomes compared to baseline weight. Duration of pre-pregnancy ART was associated with higher baseline weight for DTG, and the risk of maternal hypertension by baseline weight category was higher for DTG than EFV.

Measuring Health Outcomes in HIV: Time to Bring in the Patient Experience

Niki O'Brien et al.

Summary: Although the global response to HIV has led to a reduction in new infections and associated mortality, there is still a high number of people living with HIV. As HIV transitions to a chronic condition, it is necessary to evaluate health outcomes beyond biological markers by considering health-related quality of life. Efforts should be directed towards validating patient-reported measures to ensure that the full range of health, mental, and social outcomes related to the disease are captured.
