Adding creatinine to routine pregnancy tests: a decision tree for calculating the cost of identifying patients with CKD in pregnancy

Giorgina Barbara Piccoli et al.

Summary: This study examines the cost and benefits of early CKD diagnosis by adding serum creatinine to prenatal screening and monitoring tests. The best strategy identified is to conduct one test each before, during, and after pregnancy, which helps improve the detection rate of CKD cases.

Summary: The specific roles of remission status, lupus low disease activity state (LLDAS), and damage accrual on the prognosis of pregnancies in women with SLE are unknown. This study found that damage at conception and lupus anticoagulant were significantly associated with adverse pregnancy outcomes, while hypocomplementemia in the first trimester was associated with maternal flares later in pregnancy.

Treatment for Mild Chronic Hypertension during Pregnancy

A. T. Tita et al.

Summary: In pregnant women with mild chronic hypertension, a strategy of targeting a blood pressure of less than 140/90 mm Hg was associated with better pregnancy outcomes compared to a strategy of reserving treatment only for severe hypertension, without increasing the risk of small-for-gestational-age birth weight.

Update on the diagnosis and management of systemic lupus erythematosus

Antonis Fanouriakis et al.

Summary: Systemic lupus erythematosus (SLE) is characterized by clinical heterogeneity, unpredictable course and flares. Diagnosis remains clinical with limited serologic test confirmation, and newer classification criteria offer more accurate classification. Treatment goals focus on patient survival, flare prevention, organ damage prevention, and quality of life optimization. High-intensity immunosuppressive therapy followed by less intensive therapy is typically used to control disease activity in severe cases of SLE.

Quality or Quantity of Proteins in the Diet for CKD Patients: Does Junk Food Make a Difference? Lessons from a High-Risk Pregnancy

Alejandra Oralia Orozco-Guillien et al.

Summary: The case study highlighted the importance of considering the presence of additives in food for patients with chronic kidney disease during pregnancy. By adjusting the patient's diet to reduce animal-origin protein intake and improve food quality, there was a significant improvement in kidney function, leading to a healthy delivery at 37 weeks gestation. This emphasizes the potential negative impact of contaminants in low-quality food on kidney function and the importance of consuming healthy, unprocessed organic food, especially in delicate conditions such as pregnancy.

Summary: This study evaluated the risk of adverse fetal outcomes after exposure to ACE-Is or ARBs in the first trimester of pregnancy. Pregnancies exposed to these medications had a higher risk of major congenital malformations, cardiovascular malformations, and stillbirths compared to non-exposed pregnancies, independent of underlying maternal hypertension. Women planning for pregnancy using these medications should be counselled appropriately.

Sakviseth Bin et al.

Summary: Congenital heart block (CHB) is most commonly caused by neonatal lupus, an acquired autoimmune disease transmitted from maternal antibodies to the fetus. In this case, a full-term female neonate showed severe bradycardia at birth, later diagnosed with cardiac neonatal lupus with positive maternal anti-Ro antibodies after admission to the neonatal intensive care unit. The infant was discharged without need for a pacemaker and will have regular follow-up with a paediatric cardiologist.
