Summary: This review aimed to identify factors predicting post-thyroidectomy hypocalcemia in children and adolescents. Intraoperative parathyroid hormone may be useful in predicting hypocalcemia, but consistent clinical risk factors or preventative methods were not found in the pediatric literature. High-quality pediatric research is urgently needed to address this knowledge gap.

Improving safety in paediatric thyroidectomy by PTH measurements

Summary: Assessing patients' PTH levels to classify the risk of hypoparathyroidism and initiating preventive therapy effectively reduced the incidence of postthyroidectomy hypoparathyroid hypocalcemia. Implementing this strategy for high-risk patients significantly decreased the incidence of hypocalcemia and length of hospitalization, while routine monitoring for low-risk patients effectively controlled the occurrence of hypocalcemia.

The role of early postoperative parathyroid hormone level after total thyroidectomy in prediction of hypocalcemia

Summary: Measuring serum PTH levels 3 hours after thyroid surgery can accurately predict the development of hypocalcemia, guiding early intervention with calcium and/or vitamin D supplements for high-risk patients with PTH levels below 10 pg/ml. Patients with PTH levels above 10 pg/ml may be safely discharged early without any supplements. Further studies are needed to compare early serum PTH levels with day one serum PTH levels post total thyroidectomy to predict hypocalcemia.
