Summary: This study introduces the implementation of the Operating Room Black Box at a Canadian academic center and aims to develop evidence-based best practices for direct observation of clinical performance in the operating room using innovative technology. By overcoming various challenges at institutional, clinician, and patient levels through multiple approaches during the implementation process, the successful installation and data collection of the Operating Room Black Box were achieved.
