Usefulness of neutrophil-to-lymphocyte ratio for predicting acute pericarditis outcomes

Fatih Yilmaz et al.

Summary: NLR and hs-CRP were identified as independent predictors for the composite endpoint of tamponade and/or recurrence in acute pericarditis patients during 1-year follow-up. Higher NLR and hs-CRP levels were associated with increased risk of disease recurrence.

Recent advances in pericarditis

Emanuele Bizzi et al.

Summary: Pericardial diseases encompass a range of conditions, and recent research has shed light on their underlying mechanisms. Studies have revealed that normal pericardial fluid contains a high level of nucleated cells, proteins, albumin, and LDH, similar to inflammatory exudates found in other bodily fluids. A non-invasive wait-and-see approach may be appropriate for managing select cases of chronic idiopathic non-inflammatory pericardial effusions. In the treatment of acute recurrent pericarditis, IL-1 inhibitors have shown significant efficacy, particularly in patients with specific clinical characteristics. Multi-disciplinary teams play a crucial role in addressing the complex needs of women of childbearing age with recurrent pericarditis.

The autoinflammatory side of recurrent pericarditis: Enlightening the pathogenesis for a more rational treatment

Giuseppe Lopalco et al.

Summary: Recurrent pericarditis (RP) is a troublesome complication of acute pericarditis, with unknown etiopathogenesis. An overlap of autoimmune and autoinflammatory pathways is hypothesized to explain the origin and recurrence of RP, with the most innovative pathogenic scenario involving the overproduction of IL-1 by neutrophils and macrophages. Treatment with IL-1 blockade has shown promising results in cases refractory to NSAIDs and colchicine.

Autoimmune and Autoinflammatory Pericarditis: Definitions and New Treatments

Emanuele Bizzi et al.

Summary: The purpose of the review is to analyze the pathogenetic mechanisms of acute pericarditis, focusing on autoimmune and autoinflammatory pericarditis, and to review available treatment options. Recent studies have shown the efficacy of anti-IL-1 drugs in recurrent pericarditis, particularly in patients with raised C-reactive protein, suggesting their potential usefulness in tailored medicine for patients with a clear inflammatory phenotype.
