Contemporary Fertility-Sparing Management Options of Early Stage Endometrioid Endometrial Cancer in Young Nulliparous Patients

Gulzhanat Aimagambetova et al.

Summary: Incidence of endometrial cancer has been increasing in recent years, especially in high-income countries. Fertility-sparing treatment for young patients with EC has become extremely important due to increasing incidence among reproductive age women. Several fertility-sparing options, including medication, minimally invasive surgical techniques, and the utilization of molecular biomarkers, have been developed and found to be efficient for fertility preservation.

Biomolecular and Genetic Prognostic Factors That Can Facilitate Fertility-Sparing Treatment (FST) Decision Making in Early Stage Endometrial Cancer (ES-EC): A Systematic Review

Panayiotis Tanos et al.

Summary: Endometrial cancer has a high incidence in women under 40 years old, and the decision of fertility preservation in early stage endometrial cancer is challenging. Many patients wish to have children after successful treatment. This review identifies genetic prognostic factors that can aid in decision-making for fertility-sparing treatment in early stage endometrial cancer.

Fertility-Sparing Approaches in Atypical Endometrial Hyperplasia and Endometrial Cancer Patients: Current Evidence and Future Directions

Nayanar-Adela Contreras et al.

Summary: Endometrial cancer is the fourth most common cancer in women in developed countries. Its incidence has increased in young women, possibly due to factors such as delayed childbearing and obesity. The new molecular classification of the disease helps personalize treatment. Current evidence suggests that oral progestin alone or in combination with other drugs is a feasible and safe option for the conservative management of early-stage endometrial cancer limited to the endometrium.

Role of Leptin and Adiponectin in Endometrial Cancer

Aneta Slabuszewska-Jozwiak et al.

Summary: Obesity is strongly associated with the development of endometrial cancer, and adipokines such as adiponectin and leptin may play a role in this process. Understanding the signaling pathway of these cytokines in endometrial cancerogenesis could potentially lead to targeted therapies and fertility-sparing treatments.

Endometrial cancer

Emma J. Crosbie et al.

Summary: Endometrial cancer is the most common gynaecological cancer in high income countries, with a global rise in incidence. Obesity is the major underlying cause, posing challenges for diagnosis and treatment. Early presentation with postmenopausal bleeding ensures cure, but advanced cases have poor prognosis. Minimally invasive surgical staging and targeted chemotherapeutic strategies are important advances.

Three-dimensional transvaginal ultrasound vs magnetic resonance imaging for preoperative staging of deep myometrial and cervical invasion in patients with endometrial cancer: systematic review and meta-analysis

G. Spagnol et al.

Summary: This study evaluated and compared the diagnostic test accuracy of three-dimensional transvaginal ultrasound (3D-TVS) and magnetic resonance imaging (MRI) for deep myometrial infiltration (DMI) and cervical invasion in patients with endometrial cancer (EC). The results showed that 3D-TVS demonstrated good sensitivity and specificity for the evaluation of DMI and cervical invasion, with comparable results to MRI.

Antioxidative, Anti-Inflammatory, Anti-Obesogenic, and Antidiabetic Properties of Tea Polyphenols-The Positive Impact of Regular Tea Consumption as an Element of Prophylaxis and Pharmacotherapy Support in Endometrial Cancer

Piotr Olcha et al.

Summary: Endometrial cancer (EC) is a commonly diagnosed malignant tumor in the female reproductive system. Literature suggests that 32 genes are involved in the hereditary incidence of EC. The physiological markers of EC include disorders in the antioxidative system and inflammation. A diet rich in substances aiding the response to this disorder, particularly those suitable for lifelong consumption, is important for prophylaxis and pharmacotherapy. Tea polyphenols have proven antioxidative, anti-inflammatory, anti-obesogenic, and antidiabetic properties, making tea consumption beneficial for individuals genetically predisposed for EC.

Fertility-Sparing Approach in Patients with Endometrioid Endometrial Cancer Grade 2 Stage IA (FIGO): A Qualitative Systematic Review

Pierluigi Giampaolino et al.

Summary: Conservative treatment may be an option for patients with Stage IA G2 endometrioid endometrial cancer who desire to preserve their fertility. Evaluation of immunohistochemical markers shows promise in predicting response to conservative treatment. Further large series and randomized clinical trials are needed to confirm these results.

Endometrial Cancer in Reproductive Age: Fertility-Sparing Approach and Reproductive Outcomes

Levent Mutlu et al.

Summary: Young women diagnosed with endometrial cancer face unique challenges, but preserving fertility is possible in early and low-grade cases. Data suggest that conservative approaches with close monitoring do not compromise oncologic outcomes. For patients unable to preserve fertility, assisted reproductive technologies can help achieve fertility goals.

Fertility preservation in women with early-stage gynecologic cancer: optimizing oncologic and reproductive outcomes

Jessica Lauren Floyd et al.

Summary: For women with early-stage gynecologic cancer, fertility-sparing treatments may be an option but require careful consideration of potential risks and benefits by both patients and healthcare providers. Patients undergoing fertility-sparing therapies may experience fertility and obstetric complications, highlighting the importance of preconception counseling with high-risk obstetric specialists. Collaboration between gynecologic oncologists and fertility specialists is essential for identifying appropriate candidates for fertility-sparing treatments and helping women conceive after cancer treatment.

Fertility-sparing management for endometrial cancer: review of the literature

Simone Garzon et al.

Summary: Fertility preservation appears feasible in young patients with grade 1 EC limited to the endometrium or atypical endometrial hyperplasia. Progestins are the mainstay of such management. The addition of Metformin and hysteroscopic resection seems to provide some improvements. However, fertility preservation is not the standard approach for staging and treatment, potentially worsening oncologic outcomes.

The Perspectives of Fertility Preservation in Women with Endometrial Cancer

Jure Knez et al.

Summary: Endometrial cancer is the most common gynecological cancer in developed countries and is increasingly diagnosed in younger women of reproductive age. The standard treatment involves surgical removal, which may impact future fertility for younger women. The current challenge is to identify low-risk patients who could benefit from more conservative treatment options while focusing on preserving quality of life. Future studies should focus on identifying reliable clinical and molecular predictive markers to expand the use of fertility-preserving treatment methods.
