Summary: This study demonstrated the feasibility and practicality of using a pedicle screw classification and grading system based on computed tomography (CT) to evaluate the accuracy of cortical bone trajectory (CBT) screw placement in midline lumbar interbody fusion. Medial and foraminal perforations were associated with a higher risk of neurological deficits, especially grade 2 medial cortical perforations. Inexperienced surgeons are advised to opt for a lateral trajectory to minimize the risk of misplacement.
