The incidence, risk factors and outcomes of acute kidney injury in critically ill patients undergoing emergency surgery: a prospective observational study

Linhui Hu et al.

Summary: In a study of 383 critically ill patients undergoing emergency surgery, postoperative reoperation, postoperative APACHE II score, and postoperative serum lactic acid were identified as independent risk factors for postoperative AKI, with higher mortality rates and costs in the AKI group.

Role of perioperative hypotension in postoperative acute kidney injury: a narrative review

Yugeesh R. Lankadeva et al.

Summary: Perioperative hypotension is associated with acute kidney injury, but there are uncertainties regarding the timing and approach of interventions. Identifying safe levels of arterial pressure and conducting more clinical trials are needed to validate the effectiveness of interventions. Additionally, there is limited knowledge about the risks and benefits of different clinically available therapies for perioperative hypotension in terms of renal medullary perfusion and oxygenation.

Summary: The study found that although uNAG levels varied with FT3 and TT3 levels, these hormones did not interfere with uNAG's ability to detect AKI in patients with critical illness.

Provisional Decision-Making for Perioperative Blood Pressure Management: A Narrative Review

Qiliang Song et al.

Summary: Blood pressure is crucial for organ blood flow supply, and low blood pressure may lead to tissue hypoxia and organ injury. Intraoperative blood pressure fluctuates greatly, and hypotension is common. The causal relationship between intraoperative hypotension and organ injury is still unclear, but hypotension is definitely harmful to perfusion. Advances in machine learning and artificial intelligence may improve global hemodynamic management. There is a lack of studies on blood pressure target treatment and decision-making factors. This review aims to provide practical guidance for blood pressure treatment in the clinical setting and evaluate the benefits of an automated blood control system in predicting hypotension.

Urinary proteome analysis of acute kidney injury in post-cardiac surgery patients using enrichment materials with high-resolution mass spectrometry

Yunpeng Bai et al.

Summary: This study investigated the correlations between urinary proteomic changes and CSA-AKI. The results identified differentially expressed proteins and enriched signal pathways associated with CSA-AKI. These findings provide valuable knowledge about urinary proteome biomarkers and essential resources for further research on AKI.

Summary: The study investigated links between urinary metabolic changes and cardiac surgery-associated AKI, finding that altered renal metabolism, including sulphur and amino acid metabolism, may play a role in AKI development.

Provisional Decision-Making for Perioperative Blood Pressure Management: A Narrative Review

Qiliang Song et al.

Summary: This article discusses the relationship between blood pressure (BP) and organ injury, highlighting the dynamic changes in perioperative BP and the common issue of hypotension during surgery. Although it is unclear whether intraoperative hypotension directly causes organ injury, it is certain that it affects perfusion and causes harm to some extent. Therefore, various guidelines on intraoperative BP management have been proposed.

Acute Kidney Injury in Cardiac Surgery

Christina Massoth et al.

Summary: The incidence of acute kidney injury (AKI) after cardiac surgery is high, with about 3% of patients requiring renal replacement therapy. According to the Kidney Disease: Improving Global Outcomes (KDIGO) criteria, AKI is defined as a sudden deterioration of renal function, developing within a period of hours to days.

CSA-AKI: Incidence, Epidemiology, Clinical Outcomes, and Economic Impact

Alan Schurle et al.

Summary: CSA-AKI, a common complication following cardiac surgery, relies on the loss of renal function for diagnosis. New diagnostic injury biomarkers have shown promise in identifying renal injury risk early. Treatment is limited to supportive care, and costs are difficult to estimate accurately, but are increasing with the global rise in cardiac surgery volume.

Assessment of 17 clinically available renal biomarkers to predict acute kidney injury in critically ill patients

Yating Hou et al.

Summary: The study in the ICU patients found that serum cystatin C (CysC) had the best performance in predicting AKI and severe AKI. Combinations of CysC with N-acetyl-beta-d-glucosaminidase-to-creatinine ratio (NAG/Cr) or lactic acid (LAC) were effective for predicting AKI and severe AKI respectively. Additionally, CysC combined with clinical markers such as Acute Physiology and Chronic Health Evaluation (APACHE) II score or Multiple Organ Dysfunction Score (MODS) showed good predictive abilities for AKI and severe AKI.
