Three-Year Impact Of Stratification In The Medicare Hospital Readmissions Reduction Program

Sukruth A. Shashikumar et al.

Summary: The Medicare Hospital Readmissions Reduction Program (HRRP) penalizes hospitals with high readmission rates. A new approach has been implemented to stratify hospitals and evaluate performance within each stratum rather than in the national cohort. The impact on hospitals caring for vulnerable populations has not been studied. However, the study found that the stratification by proportion of dual enrollees was associated with penalty reductions at rural hospitals and hospitals serving Black and Hispanic or Latino patients.

Association of Stratification by Proportion of Patients Dually Enrolled in Medicare and Medicaid With Financial Penalties in the Hospital-Acquired Condition Reduction Program

Sukruth A. Shashikumar et al.

Summary: This study of 3102 hospitals participating in the HACRP found that stratification based on dually enrolled patients significantly reduced penalties and disparities, particularly benefiting safety-net hospitals. Policy makers should consider implementing similar stratification for other value-based payment programs.

Adjusting Quality Measures For Social Risk Factors Can Promote Equity In Health Care

David R. Nerenz et al.

Summary: The practice of adjusting quality measures using social risk factors remains controversial, but can be appropriate and necessary in specific circumstances. By making social risk adjustment the default option when valid empirical arguments exist, inequities within the healthcare system can be avoided.
