Clinical Manifestations and Pathogenesis of Acute Necrotizing Encephalopathy: The Interface Between Systemic Infection and Neurologic Injury

Priya Shukla et al.

Summary: Acute necrotizing encephalopathy (ANE) is a severe neurological condition that can occur following systemic infections such as influenza and SARS-CoV-2. The disease is characterized by rapid changes in consciousness, neurological deficits, and seizures. Neuroimaging reveals bilateral deep gray matter lesions, often involving the thalami, with evidence of necrosis and/or hemorrhage. Differential diagnosis includes viral infection of the central nervous system, as well as metabolic and mitochondrial disorders. The prognosis of ANE is generally poor, especially in cases involving the brainstem. Current management focuses on modulating immune responses to mitigate neurological injury.

Roles of Nucleoporin RanBP2/Nup358 in Acute Necrotizing Encephalopathy Type 1 (ANE1) and Viral Infection

Jing Jiang et al.

Summary: This article summarizes recent advances in understanding how mutations in the RANBP2 gene contribute to ANE1 and how RanBP2 interacts with distinct viruses to affect viral infection. Recent findings suggest that RanBP2 might be an important therapeutic target for the suppression of ANE1-driven cytokine storms and to combat hyperinflammation in response to viral infections.

Workshop on RanBP2/Nup358 and acute necrotizing encephalopathy

Alexander F. Palazzo et al.

Summary: This article discusses the relationship between missense mutations in RanBP2/Nup358 and Acute Necrotizing Encephalopathy (ANE), a pediatric disease characterized by a cytokine storm restricted to the central nervous system after viral infection. If left untreated, this condition can lead to seizures, coma, long-term neurological damage, and a high mortality rate. However, the exact mechanism by which RanBP2 mutations contribute to the development of ANE is still unclear. Furthermore, the article mentions that several researchers have presented their work on this disease and the interactions between RanBP2/Nup358, viral infections, the innate immune response, and other cellular processes.

Case Report: Acute Necrotizing Encephalopathy Following COVID-19 Vaccine

Mohamed Reda Bensaidane et al.

Summary: This article reports a case of acute necrotizing encephalopathy (ANE) following COVID-19 vaccination. Cytokine analysis showed elevated levels of multiple cytokines in the patient's serum and cerebrospinal fluid. Treatment with high-dose corticosteroids led to favorable outcomes. The authors speculate that the vaccine may have triggered an aberrant immune response. Further research on the pathogenesis of ANE in the context of COVID-19 vaccination is of great importance.

COVID-19 related acute necrotizing encephalopathy with extremely high interleukin-6 and RANBP2 mutation in a patient with recently immunized inactivated virus vaccine and no pulmonary involvement

Thanakit Pongpitakmetha et al.

Summary: This study reports the first case of COVID-19 associated acute necrotizing encephalopathy (ANE) without pulmonary disease in a patient with high interleukin-6 (IL-6) level and Ran Binding Protein 2 (RANBP2) mutation. The patient's clinical course was complicated and resulted in death.

Characterization of SARS-CoV-2 proteins reveals Orf6 pathogenicity, subcellular localization, host interactions and attenuation by Selinexor

Jin-Gu Lee et al.

Summary: This study identified Orf6 as a highly pathogenic protein in the SARS-CoV-2 genome and its interaction with key host proteins. The drug Selinexor was found to attenuate Orf6-induced cellular toxicity, suggesting it as a potential candidate for targeting Orf6 host protein interactions that lead to cytotoxicity.
