In a rapidly changing health and care landscape, it is important to acknowledge the need for integrated care with a patient-centered approach. Unfortunately, care systems are often fragmented and focused on the condition rather than the patient. To address this issue, the Science Committee of the Association of Cardiovascular Nursing and Allied Professions of the European Society of Cardiology has issued a statement on integrated care for those with cardiovascular disease. This statement outlines the evidence, challenges, and recommendations for practice, education, and research.
