Summary: The last international guidelines on the evaluation and management of primary hyperparathyroidism (PHPT) were published in 2014. New research since then has led to new insights in epidemiology, pathophysiology, diagnosis, measurements, genetics, outcomes, presentations, new imaging modalities, target and other organ systems, pregnancy, evaluation, and management. An international group of experts reviewed these advances in knowledge and provided evidence-based recommendations and guidelines for the evaluation and management of PHPT.

Health-Related Quality of Life in Patients with Multiple Endocrine Neoplasia Type 1

Summary: Patients with MEN1 have lower health-related quality of life scores compared to the general Dutch population, with employment status being the most consistent predictor for HRQOL, followed by the presence of a pituitary tumor. Some patients with pNET or pituitary tumors were unaware of their condition, and these patients had better QOL scores compared to those who were aware of their tumors. These findings highlight the importance of addressing the impaired QOL in MEN1 patients in routine care.

Neuropsychologic changes in primary hyperparathyroidism after parathyroidectomy from a dual-institution prospective study

Summary: The study showed significant improvement in neuropsychiatric symptoms after parathyroidectomy, with these symptoms being more prevalent in patients with primary hyperparathyroidism. This suggests that neuropsychiatric symptoms should be carefully evaluated in all patients with primary hyperparathyroidism and be considered as a relative indication for parathyroidectomy.

Summary: This study found that the QoL of patients with PHPT significantly improved 3 months and 3 years after successful parathyroidectomy, while the QoL of the conservative treatment group continued to deteriorate over 3 years. Parathyroidectomy had a significant positive impact on improving QoL.

Prediction of cognitive response to surgery in elderly patients with primary hyperparathyroidism

Summary: The study aimed to evaluate the impact of short-term medical normalization of hypercalcemia on surgical decision-making for elderly patients with mild cognitive deficiency. Results showed that calcimimetic treatment led to improvements in MoCA and HADS scores, and long-term increase in MoCA score correlated with the decrease in ionized calcium concentration. Both baseline calcium concentration and improvement in MoCA with calcimimetic treatment were identified as independent predictors of favorable outcome after parathyroidectomy.
