Technology Utilization in Black Adolescents with Type 1 Diabetes: Exploring the Decision-Making Process

Summary: This study aimed to understand the decision-making process and influencing factors for the use of diabetes devices (DDs) in Black adolescents and their parents. The results showed that Black adolescents face challenges related to the intersectionality of social identities, the complexity of T1D management, and differential uptake of DDs. Cultural, familial, and individual factors were found to be both facilitators and barriers in DD use. Lack of familiarity with T1D, limited exposure to DDs, and mistrust of the medical community were identified as reasons for inequities in DD use.

Advanced Diabetes Technology Remains Underutilized in Underserved Populations: Early Hybrid Closed-Loop System Experience at an Academic Safety Net Hospital

Summary: Retrospective evaluation of outcomes using the Minimed Medtronic 670G system in a group of diabetes patients found that those who initiated auto-mode showed significant improvement in A1c levels after 1 year, while 31% of patients never initiated auto-mode. Additionally, disparities were observed among black and Hispanic patients, who comprised 50% of the group and exhibited some barriers to technology use. Future studies are needed to identify barriers and strategies for increasing the use of advanced insulin delivery systems in underserved populations.

A Decade of Disparities in Diabetes Technology Use and HbA1c in Pediatric Type 1 Diabetes: A Transatlantic Comparison

Summary: This study examined the impact of socioeconomic status on diabetes technology use and glycemic control in youth. The results showed that in the T1DX registry in the United States, participants with lower socioeconomic status had higher levels of glycated hemoglobin, and the gap in HbA(1c) by SES widened over the past decade; whereas in the DPV registry in Germany, the influence of SES on technology use and glycemic control was weaker.

Summary: There are racial/ethnic disparities in the use of diabetes technology among adults with type 1 diabetes in urban safety-net hospitals, with non-White patients, particularly Black and other race/ethnicity, being significantly less likely to use continuous glucose monitoring (CGM) and continuous subcutaneous insulin infusion (CSII) compared to White patients. However, the use of any diabetes technology is associated with better glycemic control, with the combination of CGM and CSII showing the lowest levels of hemoglobin A1c across all racial/ethnic groups. Further research is needed to explore barriers to accessing diabetes technology in non-White populations.

The study found that lower CGM use in non-Hispanic Black children was due to lower rates of device initiation and higher rates of discontinuation.
