We present Ga-68-DOTA-FAPI-04 and F-18-FDG PET/CT findings of intracranial syphilitic gumma in a 46-year-old man with HIV. In this case, Ga-68-DOTA-FAPI-04 PET/CT outperforms F-18-FDG in helping to visualize syphilitic gumma. Syphilitic gumma can also cause increased FAPI activity. Our findings suggest the potential value of Ga-68-DOTA-FAPI-04 in the diagnosis of syphilis.
