Nocardia Infection With Adrenal Gland Abscess Mimicking Metastatic Lung Cancer on FDG PET/CT

Summary: We present the FDG PET/CT findings of a HIV-positive patient suspected of having lung cancer with adrenal gland metastasis. Wedge resection of the pulmonary nodules revealed Nocardia infection, and subsequent FDG PET/CT imaging after antibiotic treatment showed complete metabolic response of the adrenal lesion and pulmonary nodules. It is important to consider that nocardiosis can present as FDG-avid lesions mimicking malignancies in immunocompromised patients.
