Role of occupational therapy in epilepsy patients after left temporal lobe surgery

Summary: There is a relevant risk of psychosocial and cognitive impairments in epilepsy patients with resective surgery in the left temporal lobe. Surgery in the speech-dominant hemisphere can lead to deterioration of speech-related functions, including verbal memory. Limited studies have addressed the impact of occupational therapy in postoperative rehabilitation of epilepsy patients. This study shows that occupational therapy can be an efficient tool in the treatment of psychosocial and cognitive impairments after epilepsy surgery.

Roles of fMRI and Wada tests in the presurgical evaluation of language functions in temporal lobe epilepsy

Summary: Surgical treatment for pharmacoresistant temporal lobe epilepsy carries risks for language function. Predicting language function decline before surgery is as important as predicting seizure-free outcomes. The Wada test (WT) has traditionally been used for language lateralization, but it is invasive and does not localize language function. Functional magnetic resonance imaging (fMRI) has great potential as a noninvasive alternative, allowing localization and predictive value for postsurgical outcomes. This review focuses on the importance of language evaluation and the role of fMRI and WT in presurgical assessments.
