Triple post transplant cyclophosphamide (PTCY) based GVHD prophylaxis: HLA matched versus HLA haploidentical transplants

Summary: In this retrospective analysis of allogeneic hematopoietic stem cell transplant recipients, the use of post-transplant cyclophosphamide, cyclosporine, and mycophenolate mophetil as graft-versus-host-disease prophylaxis showed differences in outcomes between HLA-matched and haploidentical grafts, with higher rates of acute and chronic GVHD in the HLA-mismatched group. However, disease-free survival was comparable between the two groups.

Clinical Significance of Haplo-Fever and Cytokine Profiling After Graft Infusion in Allogeneic Stem Cell Transplantation From Haplo-Identical Donors

Summary: This study retrospectively analyzed the characteristics of haplo-fever and cytokine profiles in patients undergoing haplo-HSCT. It found that patients with high peak temperatures had a lower incidence of chronic GvHD and better GvHD and relapse-free survival rates. Multiple cytokines, including IL2 receptor and TNF, significantly increased during haplo-fever, and their increases were correlated with a lower risk of cGvHD. Increased TNF > 1.8055-fold on day +4 was the best threshold for predicting cGvHD.

Summary: A comparison was made between ATG and PTCy-based GvHD prophylaxis in 434 adults with acute lymphoblastic leukemia undergoing haploidentical hematopoietic cell transplantation, showing that PTCy resulted in a lower relapse incidence, improved leukemia-free survival, and overall survival compared to ATG.

HLA-haploidentical vs matched unrelated donor transplants with posttransplant cyclophosphamide-based prophylaxis

Summary: After comparing outcomes of 2036 haploidentical and 284 matched unrelated donor (MUD) transplantations with posttransplant cyclophosphamide (PTCy) graft-versus-host disease (GVHD) prophylaxis, it was found that MUD transplants had lower acute GVHD and nonrelapse mortality rates, as well as higher disease-free and overall survival, especially in recipients of reduced-intensity regimens. However, no significant differences were observed in patients receiving myeloablative conditioning regimens, highlighting the importance of donor-recipient HLA matching for allogeneic transplantation.
