Methodological issues in research on drug-related admissions: A meta-epidemiological review with focus on causality assessments

30-Day Potentially Avoidable Readmissions Due to Adverse Drug Events

Half of them were due to prescription errors, mainly involving diuretics, analgesics, or antithrombotics, and the other half were due to suboptimal patient monitoring/education, most frequently with antineoplastics.
