The Effectiveness of Transitional Care Interventions on Health Care Utilization in Patients Discharged From the Hospital With Heart Failure: A Systematic Review and Meta-Analysis

Summary: This study evaluated the effectiveness of transitional care interventions on healthcare utilization after hospitalization for heart failure. The results showed a reduction in all-cause and HF-specific readmissions, but not in ED visits. Multidisciplinary interventions were found to be most effective in reducing HF readmissions, with an inverse linear dose-response relationship between intervention intensity and complexity with HF readmission risk.
