Coronary plaque composition influences biomechanical stress and predicts plaque rupture in a morpho-mechanic OCT analysis

Andrea Milzi et al.

Summary: By utilizing finite-element analysis based on OCT-generated plaque models, this study found that ruptured plaques had significantly higher maximal stress within the fibrous cap and vessel wall compared to non-ruptured plaques. Ruptures tended to occur in the vicinity of maximal stress concentrations, and stress concentrations were excellent predictors of plaque rupture, surpassing established vulnerability features like fibrous cap thickness or macrophage infiltration.
