Local Failure Events in Prostate Cancer Treated with Radiotherapy: A Pooled Analysis of 18 Randomized Trials from the Meta-analysis of Randomized Trials in Cancer of the Prostate Consortium (LEVIATHAN)

Summary: The study aimed to evaluate the prognostic impact of local failure and the dynamics of distant metastasis following radiotherapy. The results showed that local failure was an independent prognosticator of overall survival, prostate cancer-specific survival, and distant metastasis-free survival in high-risk prostate cancer patients, and it was significantly associated with distant metastasis-free survival in intermediate-risk patients. Additionally, the study found that a "second wave" of distant metastases occurred subsequent to local failure events.

Salvage radical prostatectomy after local radiotherapy in prostate cancer

Summary: Salvage radical prostatectomy is mainly performed in high volume centres, with increasing data on minimal invasive surgery and conflicting data on functional outcome. As patients with prostate cancer have longer life expectancy even in metastasized disease, the possibility of developing symptomatic progression increases. There are surgical approaches to palliate these patients sufficiently, and salvage radical prostatectomy is a definitive curative option to avoid local progression.
