Predicting Positive Margins in Pancreatic Head Adenocarcinoma After Neoadjuvant Therapy: Investigating Disparities in Quality Care Using the National Cancer Database

Corey Suraci et al.

Summary: This study analyzed over 4000 patients with pancreatic head adenocarcinoma who received pancreaticoduodenectomy, and found that male sex, larger tumor size, and positive lymph nodes were predictors of positive margins after resection with neoadjuvant therapy. Outlier facilities with higher than expected rates of positive margins were associated with lower yearly hospital volumes, indicating a disparity in care quality.

R Status is a Relevant Prognostic Factor for Recurrence and Survival After Pancreatic Head Resection for Ductal Adenocarcinoma

Stefano Crippa et al.

Summary: The presence of positive resection margins, especially R1 direct and R1 <= 1 mm definitions, is an independent predictor of disease-free survival and disease-specific survival in patients with PDAC. The involvement of multiple margins increases the risk of cancer recurrence and poor survival outcomes. Different surgical margins may have different prognostic roles in PDAC.

Improved survival after pancreatic re-resection of positive neck margin in pancreatic cancer patients. A systematic review and network meta-analysis

Stefano Crippa et al.

Summary: The oncological benefit of achieving a negative pancreatic neck margin through re-resection after a positive frozen section (FS) is debated. Results suggest that a negative neck margin after primary resection or re-resection of a positive FS is associated with improved survival outcomes for patients with ductal adenocarcinoma.

Impact of Margin Status on Survival in Patients with Pancreatic Ductal Adenocarcinoma Receiving Neoadjuvant Chemotherapy

Ryan K. Schmocker et al.

Summary: The study suggests that microscopic margin positivity is not associated with recurrence and survival in localized PDAC patients resected after treatment with neoadjuvant chemotherapy. Factors such as clinical stage, duration of NAC, nodal status, histopathologic treatment response score, and receipt of adjuvant chemotherapy are associated with overall survival.

Nerve fibers in the tumor microenvironment in neurotropic cancer-pancreatic cancer and cholangiocarcinoma

Xiuxiang Tan et al.

Summary: PDAC and CCA are deadly cancers that share many biological features, including perineural invasion. This review discusses the interaction between nerve fibers and the tumor microenvironment, immune cells, and fibroblasts, highlighting shared signaling pathways and potential therapeutic targets. Nerve fibers may serve as powerful biomarkers for prognosis and guide personalized care in these neurotropic cancers.

Contemporary Reappraisal of Intraoperative Neck Margin Assessment During Pancreaticoduodenectomy for Pancreatic Ductal Adenocarcinoma A Review

Jashodeep Datta et al.

Summary: The management of patients with microscopically positive intraoperative neck margin (IONM) during pancreaticoduodenectomy for localized pancreatic ductal adenocarcinoma (PDAC) remains controversial. Studies have shown conflicting results on whether additional resection to achieve a tumor-free margin influences survival in IONM-positive cases, with some suggesting no impact and others indicating potential improvement in survival with additional resection or total pancreatectomy. Further research is needed to explore the relationship between margin status, tumor biology, and response to chemotherapy.

Predictive factors for long-term survival after surgery for pancreatic ductal adenocarcinoma: Making a case for standardized reporting of the resection margin using certified cancer center data

Dirk Weyhe et al.

Summary: Factors affecting overall survival after pancreatic ductal adenocarcinoma include nodal status, chemotherapy administration, UICC staging, and resection margin, with discrepancies in defining tumor free resection margin complicating survival analysis. Multivariate analysis showed that long-term survival is associated with chemotherapy, nodal status, and resection margin, rather than just initial tumor characteristics. Standardized histopathological reporting, including resection margin size, is crucial for accurate prognosis and treatment planning.

Effect and limitation of neoadjuvant chemotherapy for pancreatic ductal adenocarcinoma: consideration from a new perspective

Yoshihiro Kurata et al.

Summary: Neoadjuvant chemotherapy may play a role in downstaging and improving prognosis in a small subset of tumors. However, postoperative prognosis may be influenced by the pathologic stage of resected specimen, and NAC may be applicable to borderline resectable and locally advanced PDAC.

The Role of Pathological Method and Clearance Definition for the Evaluation of Margin Status after Pancreatoduodenectomy for Periampullary Cancer. Results of a Multicenter Prospective Randomized Trial

Gennaro Nappo et al.

Summary: There is no clear evidence on the most effective method of pathological analysis and clearance definition for defining R1 resection after PD. The 1 mm clearance is the most effective factor in determining the R1 rate after PD, but the pathological method is crucial in accurately evaluating its prognostic role.

Technical advances in surgery for pancreatic cancer

M. Schneider et al.

Summary: This review summarizes technical advances in surgery for locally advanced pancreatic ductal adenocarcinoma (PDAC), including new approaches like artery-first and uncinate-first methods, as well as techniques for resection and reconstruction to enhance completeness and reduce local recurrence. Advanced surgical techniques provide a toolkit for curative-intent surgery in PDAC, even in cases with arterial or venous involvement, but effects on overall survival still require high-level clinical evidence.
