Circulating immune-complexes and complement activation through the classical pathway in myeloperoxidase-ANCA-associated glomerulonephritis

Summary: The study suggests that complement activation through the classical pathway induced by immune complexes is involved in the pathogenesis of MPO-AAGN. CICs, C5a, and C5b-9 were significantly increased in MPO-AAGN patients and correlated with each other. Immunofluorescence staining showed deposition of C4d, C5, and C5b-9 in the glomeruli of MPO-AAGN patients, while the deposition of MASP-1, MASP-2, MBL, and factor Bb was minimal.

Diagnosis remains clinical with limited serologic test confirmation, and newer classification criteria offer more accurate classification. Treatment goals focus on patient survival, flare prevention, organ damage prevention, and quality of life optimization. High-intensity immunosuppressive therapy followed by less intensive therapy is typically used to control disease activity in severe cases of SLE.

Neural cell adhesion molecule 1 is a novel autoantigen in membranous lupus nephritis

Summary: Research has identified neural cell adhesion molecule 1 (NCAM1) as a target antigen in some cases of membranous lupus nephritis and rare cases of primary membranous nephropathy, colocalizing with IgG in glomerular immune deposits. Circulating antibodies to NCAM1 have been detected in serum of patients with NCAM1-associated membranous nephropathy, suggesting diagnostic significance. Future studies are needed to determine whether anti-NCAM1 antibody levels correlate with disease activity or response to therapy.

Avacopan for the Treatment of ANCA-Associated Vasculitis

Summary: In this study, the C5a receptor inhibitor avacopan was found to be noninferior to prednisone taper in achieving remission at week 26, but superior in achieving sustained remission at week 52 in patients with ANCA-associated vasculitis. All patients received cyclophosphamide or rituximab. The safety and clinical effects of avacopan beyond 52 weeks were not addressed in the trial.

Summary: Patients with MPO-ANCA GN who had immune complex deposits in the kidney showed lower platelet count, lower sC3 and sIgG levels, and higher serum creatinine levels. Additionally, patients with low sC3 levels at the initial diagnosis and during treatment displayed a trend toward poorer kidney survival, indicating the potential benefit of complement targeted therapies in AAV.
