Incidence of small-for-gestational-age infant birthweight following early intertwin fetal growth discordance in dichorionic and monochorionic twin pregnancies

Liberty G. Reforma et al.

Summary: This retrospective cohort study aimed to compare the rates of small-for-gestational-age infants among twin pregnancies with intertwin growth discordance in the absence of fetal growth restriction with that among twin pregnancies with concordant, normal growth identified by ultrasound between 24 0/7 and 31 6/7 weeks' gestation. The results showed that regardless of chorionicity, twin pregnancies with discordance without fetal growth restriction were nearly twice as likely to develop small-for-gestational-age neonates, deliver earlier in gestation, and experience greater neonatal morbidity than twin pregnancies without discordance. This emphasizes the importance of recognizing intertwin growth discordance in twin pregnancies and provides information on adverse perinatal outcomes.

Placental hypoplasia and maternal organic vascular disorder in pregnant women with gestational hypertension and preeclampsia

Yuki Owaki et al.

Summary: This study explored the etiology and pathology of preeclampsia (PE) and gestational hypertension (GH). Severe placental hypoxic changes and oxidative DNA damage were found in patients with PE, along with increased antiangiogenic factors. Maternal organic vascular disorder was more severe in GH patients compared to PE patients, as assessed by IMT.

A preeclampsia risk prediction model based on maternal characteristics and serum markers in twin pregnancy

Young Ran Kim et al.

Summary: In twin pregnancy, a risk prediction model combining maternal age, BMI, uE(3), and inhibin A was found to be a better predictor for preeclampsia. Significant differences were observed in gestational age and placenta weight between patients with and without preeclampsia, and the inhibin A level also showed high predictive value.

Patterns of discordant growth and adverse neonatal outcomes in twins

Liran Hiersch et al.

Summary: This study classified distinct growth patterns of discordant fetal growth in twin pregnancies and found that these patterns were associated with adverse pregnancy outcomes. The early progressive discordance group showed the strongest association with preterm birth and preeclampsia, while the late discordance group had the weakest association.

Prediction of pre-eclampsia in twin pregnancy by maternal factors and biomarkers at 11-13 weeks' gestation: data from EVENTS trial

Z. Benko et al.

Summary: This study aimed to validate a previous model for screening for pre-eclampsia in twin pregnancies, compare biomarker distributions between twin and singleton pregnancies and develop new screening models. The performance of the new models varied by gestational age and included factors such as maternal characteristics, MAP, UtA-PI, and PIGF.

Multifetal Gestations: Twin, Triplet, and Higher-Order Multifetal Pregnancies ACOG Practice Bulletin, Number 231

Edward J. Hayes

Summary: The incidence of multifetal gestations in the United States has significantly increased in the past few decades, especially for twin and triplet and higher-order multifetal gestations. This increase is partly attributed to older maternal age at conception and the widespread use of assisted reproductive technology. Multiple gestations are associated with various perinatal complications, with one of the most serious being preterm birth and its related infant health issues.
