Summary: There is a strong association between nonalcoholic fatty liver disease (NAFLD) and heart failure (HF), with higher severity of NAFLD being associated with an increased risk of new-onset HF. Patients with NAFLD may develop accelerated coronary atherosclerosis, myocardial alterations, and certain arrhythmias, which may precede and promote the development of HF.

Summary: This study aimed to understand the current practices, needs, and potential barriers to implementing optimal care for patients with atrial fibrillation (AF). The surveys found that standardized protocols and care pathways were not commonly used in emergency departments or inpatient settings. All stakeholders agreed that stroke prevention was a top priority, but bleeding risk was the most frequent barrier to initiating treatment. Patients emphasized the importance of education on modifiable causes and reducing stroke risk. Less than half of the healthcare systems tracked AF patients or treatment status. The most common barriers to implementing AF centers of excellence were administrative support and cost.

Cardiovascular events after liver transplantation: MACE hurts

Mario H. Altieri et al.

Summary: This review discusses the factors that contribute to the development of major adverse cardiovascular events (MACE) after liver transplantation and explores methods for predicting these events in the post-transplant period. It reviews the roles of metabolic syndrome, renal dysfunction, non-alcoholic fatty liver disease, diagnostic tests such as imaging and biomarkers, as well as parameters such as systolic and diastolic dysfunction, and QT interval prolongation in cardiovascular events. The review also summarizes the current literature on scoring systems for predicting cardiovascular events.

Anticoagulation in thrombocytopenic patients - Time to rethink?

Jecko Thachil et al.

Summary: One of the challenges in the anticoagulation era is determining how to safely administer these medications to patients with low platelet counts. Current practice relies on using a platelet count threshold to assess bleeding risk, but this approach lacks strong evidence. In this forum article, the authors discuss the evaluation of bleeding risk in thrombocytopenic patients and propose potential markers beyond platelet counts to aid in this assessment. Future studies should investigate combinations of these markers to guide the use of anticoagulation in patients with severe thrombocytopenia.

Repetitive amiodarone administration causes liver damage via adipose tissue ER stress-dependent lipolysis, leading to hepatotoxic free fatty acid accumulation

Einav Hubel et al.

Summary: Repetitive administration of amiodarone can induce hepatic lipid accumulation through mechanisms such as hepatic ER stress, impaired β-oxidation, and inhibition of triacylglycerol secretion. The study also shows that repetitive amiodarone treatment can lead to both hepatic and adipose tissue ER stress, lipolysis, hepatic accumulation of free fatty acids, and enrichment of palmitate in total lipids. Understanding the toxic mechanisms of amiodarone could help in devising strategies to limit liver damage.
