Measuring Peak Inspiratory Flow in Patients with Chronic Obstructive Pulmonary Disease

Jill A. Ohar et al.

Summary: This article evaluates the recommended and applied approaches for measuring peak inspiratory flow (PIF) in dry powder inhalers (DPIs). It highlights the lack of practical and standardized recommendations and the considerable variation in measuring PIF in clinical and real-life studies. The authors propose specific recommendations to standardize the PIF measurement process and ensure accurate and reliable results in clinical trials and daily practice.

In-vivo tongue stiffness measured by aspiration: Resting vs general anesthesia

K. D. R. Kappert et al.

Summary: This study aimed to investigate the impact of tongue cancer treatment on speech, swallowing, and mastication functions by simulating treatment effects and measuring the mechanical behavior of the tongue. The results showed that the stiffness of the tongue under general anesthesia was unexpectedly 2 to 2.5 times higher than at rest, indicating the potential effects of anesthesia on tongue properties.

Effect of inflow conditioning for dry powder inhalers

Gajendra Singh et al.

Summary: This study investigated the transport of pharmaceutical dry powder inside an inhaler-like device using high-speed imaging, focusing on the effect of inflow modifications on the dispersion characteristics of agglomerates inside the device. By strategically perturbing airflow conditions, the research observed complete fluidisation of the powder bed and various behaviors such as local turbulence, bimodal fragment size behavior, and flow separation induced by changes in flow behavior. The study's findings provide valuable data for understanding the impact of inflow conditions on a dry powder inhaler-like device and validating computational models.

Gaining an insight into the importance of each inhalation manoeuvre parameter using altered patients' inhalation profiles

Mohamad Abadelah et al.

Summary: Inhalation parameters, particularly the peak inhalation flow, have a significant impact on the dose emission and aerodynamic particle size distribution of indacaterol Breezhaler. Achieving an optimal inhalation manoeuvre combining the highest peak inhalation flow, inhalation acceleration rate, and inhaled volume in one single inhalation would improve both the quantity and quality of the aerosol.

Inspiratory Flow Parameters Through Dry Powder Inhalers in Healthy Volunteers and Patients with Chronic Obstructive Pulmonary Disease (COPD): Device Resistance Does Not Limit Use in COPD

Rain Jogi et al.

Summary: The study on inhalers in COPD patients and healthy volunteers showed similar inspiratory flow parameters between the two groups, with the majority of patients being able to correctly use the inhalers. Easyhaler was favored by 51% of patients, and learning to use both inhalers was very easy for almost 70% of patients.

Dry Powder Inhalers in the Digitalization Era: Current Status and Future Perspectives

Styliani Xiroudaki et al.

Summary: The term drug delivery systems has replaced previous terms like dosage forms to emphasize the importance of drug carriers, but may not always be fully justified in the case of delivery devices, which require a deep understanding of the complex formulation-device-patient interplay. Despite progress in particle engineering and device design, dry powder inhalers' clinical performance is limited by variable patients' breathing patterns, prompting the need for next-generation DPIs to adapt to individual respiratory capacity. The recent wave of digitalization in healthcare and industry may drive DPI technology towards personalized device-formulation-patient liaison, aligning novel technologies with a systems theory approach.
