Applying the hip-spine relationship in total hip arthroplasty

Daniel H. Wiznia et al.

Summary: This article discusses the importance of understanding the hip-spine relationship to prevent dislocations following total hip arthroplasty within Lewinnek's anatomical safe zone. By simplifying definitions and principles, identifying and treating high-risk hip-spine patients, and discussing common pathologies, orthopaedic professionals can better manage these cases for improved patient outcomes.

Functional Anatomy of the Hip Joint

Nathanael Heckmann et al.

Summary: The study aimed to define the functional anatomy of the hip joint using clinical and radiographic analyses. It was found that pelvic motion is coordinated with hip motion, with specific ratios observed between various motions. The triangle model visually illustrates the coordinated mobility of the hip joint during postural changes.

Dislocation rate and its risk factors in total hip arthroplasty with concurrent extensive spinal corrective fusion with pelvic fixation for adult spinal deformity

Hiroki Furuhashi et al.

Summary: This study retrospectively analyzed data of 23 adults who underwent extensive spinal corrective fusion with pelvic fixation and THA. The rate of THA dislocations was extremely high, with all dislocations occurring posteriorly in patients with prior THAs that were performed using the posterior approach. Pelvic tilt and cup radiographic anteversion were identified as significant factors associated with dislocations.

Does timing of lumbar fusion affect dislocation rate after total hip arthroplasty?

Gerald Andah et al.

Summary: This study compared hip dislocation rates in patients undergoing primary total hip arthroplasty (THA) before or after lumbar spinal fusion (LSF). Those who had THA following LSF had higher rates of hip dislocation, with decreased lumbar lordosis and increased pelvic incidence-lumbar lordosis mismatch observed in dislocators.

The Effect of Functional Pelvic Tilt on the Three-Dimensional Acetabular Cup Orientation in Total Hip Arthroplasty Dislocations

Thom E. Snijders et al.

Summary: In this study, it was found that posterior THA dislocations showed lower increases in three orientation angles from standing to sitting compared to stable THAs, resulting in decreased sitting coronal inclination and sagittal ante-inclination, which may increase the risk of impingement due to THA instability. The importance of biplanar data on functional cup orientation was emphasized.

True Cumulative Incidence of and Risk Factors for Hip Dislocation within 2 Years After Primary Total Hip Arthroplasty Due to Osteoarthritis A Nationwide Population-Based Study from the Danish Hip Arthroplasty Register

Lars L. Hermansen et al.

Summary: The true 2-year cumulative incidence of dislocation after primary THAs performed in Denmark between 2010 and 2014 was found to be 3.5%. Age, sex, ASA score, head size and type, fixation method, and surgical approach were identified as independent significant factors for dislocation. Comprehensive search algorithms are needed to accurately report complications in national registers.
