Human error identification and risk prioritization in LPG unloading operations

Summary: This study aims to develop a method for identifying human errors and prioritizing the associated risks in a liquefied petroleum gas unloading operation. The methodology used hierarchical task analysis and a systematic human error reduction and prediction approach to successfully identify and categorize errors in each task. Fuzzy failure mode and effect analysis were employed to quantify the risk of each identified error. Finally, the VIseKriterijumska Optimizacija I Kompromisno Resenje method was used to rank and prioritize the risks associated with each error.
