The Role of Endothelial Related Circulating Biomarkers in COVID-19. A Systematic Review and Meta-analysis

Summary: This study found a correlation between circulating cell-free DNA (cfDNA) levels and the development of acute kidney injury (AKI) in COVID-19 patients. cfDNA levels were significantly higher in those with severe AKI and those requiring renal replacement therapy. Furthermore, cfDNA was positively correlated with inflammatory and thrombotic biomarkers and negatively correlated with certain anticoagulant and anti-inflammatory markers.

Summary: This study evaluated the link between gut barrier defects and neutrophil extracellular traps (NETs) in coronavirus disease 2019 (COVID-19) using blood samples from COVID-19 cases and healthy controls. The results showed that specific serum markers and NETs parameters can differentiate the severity of COVID-19. Furthermore, pathogen molecules from the gut and neutrophil involvement play a significant role in the NETs-induced hyperinflammation in COVID-19.

Summary: The incidence of thrombotic events is higher in COVID-19 patients, with venous thrombotic events being more common. Elevated levels of D-dimer are associated with thrombotic events and can be used for early detection. Thrombotic events are associated with increased risk of intubation/90-day mortality. Some thrombotic events occur in patients receiving thromboprophylactic treatments. SARS-CoV-2 infection and sedentary lifestyle may play a more crucial role in the risk of thrombosis.

Long COVID endotheliopathy: hypothesized mechanisms and potential therapeutic approaches

Summary: SARS-CoV-2 infected individuals may suffer from a multi-organ disorder known as "long COVID" or post-acute sequelae of SARS-CoV-2 infection (PASC). The pathophysiology is unknown and there are no standard treatments available. Acute COVID-19 and PASC have different clinical characteristics, with acute COVID-19 being associated with systemic inflammation, hypercoagulability, and comorbidities, while these features are less prominent in PASC. The involvement of microvascular endotheliopathy and autoimmune responses in PASC is suggested, along with the reactivation of latent pathogens and microvascular thrombosis.

Levels of DEFA1, S100A8/A9, and MPO returned to normal in recovering patients, and DEFA1 levels were higher in severe patients than in mild patients at the onset of the disease.
