Current Issues in the Use of Opioids for the Management of Postoperative Pain A Review

Pamela E. Macintyre et al.

Summary: The opioid crisis resulted from uncontrolled prescribing practices, with origins in North America and spreading to other high-income countries. Multifactorial causes include intentions to alleviate postoperative pain. Patient function assessment may be better than numerical pain scores for guiding postoperative analgesia.

Perioperative pain management for thyroid and parathyroid surgery: A systematic review

Joana E. Ochoa et al.

Summary: There is evidence suggesting that surgeons have historically over-prescribed opioid pain medications after thyroid and parathyroid surgery, potentially contributing to the US opioid epidemic. This review explores the evidence supporting multimodal methods of pain control after cervical endocrine surgery. The findings indicate limited data on pain scores following discharge and higher intervention doses compared to clinical practice. However, incorporating evidence-based, non-opioid interventions into pain management protocols may improve postoperative pain control after cervical endocrine surgery.

Risk of Complications in Patients Undergoing Completion Thyroidectomy after Hemithyroidectomy for Thyroid Nodule with Indeterminate Cytology: An Italian Multicentre Retrospective Study

Gian Luigi Canu et al.

Summary: The study aimed to assess the rate of complications in patients undergoing a completion thyroidectomy after a hemithyroidectomy for a thyroid nodule with an indeterminate cytology. There was no significant difference in terms of complications between patients undergoing completion thyroidectomy after hemithyroidectomy and those undergoing primary thyroid surgery.

Total Versus Completion Thyroidectomy: A Multidimensional Evaluation of Long-Term Vocal Alterations

Antonio Minni et al.

Summary: Total thyroidectomy (TT) and completion thyroidectomy (CT) can lead to vocal alterations, even without nerve injury. CT carries a higher risk of external branch of superior laryngeal nerve injury and results in worse vocal parameters and Voice Handicap Index scores compared to TT. Routine comprehensive functional voice analysis before and after surgery is important to understand factors contributing to vocal alterations.

Completion Thyroidectomy is Less Common Following Updated 2015 American Thyroid Association Guidelines

Lindsay E. Kuo et al.

Summary: Following the 2015 ATA guidelines, there was a significant 48.6% decrease in the rate of completion thyroidectomy for low-risk patients with DTC between 1 and 4 cm in size. However, 25% of these patients still underwent CT, indicating additional factors influencing the decision for further treatment.

A specific enhanced recovery protocol decreases opioid use after thyroid and parathyroid surgery

Linwah Yip et al.

Summary: This study showed that the enhanced recovery protocol specific for thyroid/parathyroid surgery significantly reduced perioperative opioid use and decreased the occurrence of postoperative nausea and vomiting. Patients who used the enhanced recovery protocol had a much higher rate of being entirely opioid-free after surgery.

Trends in Thyroid Surgery and Guideline-Concordant Care in the United States, 2007-2018

Asmae Toumi et al.

Summary: The study found a significant changepoint occurring in November 2015, affecting operative and management trends for benign and malignant thyroid disease. Following this, the proportion of total thyroidectomies decreased from 80% in September 2015 to 39% by December 2018 compared to thyroid lobectomies. For thyroid cancer patients, the percentage of thyroid lobectomies over total thyroidectomies has been increasing, from 17% in 2015 to 28% by the end of 2018.

Postoperative hypoparathyroidism after completion thyroidectomy for well-differentiated thyroid cancer

Davide Giordano et al.

Summary: This retrospective study found that patients undergoing completion thyroidectomy had a lower risk of transient postoperative HPT, with female sex and PGRIS score identified as independent risk factors for postoperative HPT. These findings could help in surgical planning to reduce endocrine complications, especially with the increasing use of conservative surgery for thyroid neoplasms.

Summary: This study found that multifocality was significantly associated with an increased risk of recurrence in patients with PTC, while cancer-specific survival showed no difference. Differences in tumor size, number of tumor foci, and patient age should be considered when interpreting the multifocality and the risk of recurrence.
