Role of endoscopic ultrasound-guided liver biopsy: a Meta-analysis

Keyu Zeng et al.

Summary: This meta-analysis evaluated the value of endoscopic ultrasound-guided liver biopsy (EUS-LB) for liver parenchymal and focal lesions, as well as the impact of needle-related factors and specimen adequacy criteria. The results showed that EUS-LB is an effective and safe method for liver biopsy. Acquire needles provided better specimens compared to SharkCore needles. FNB needles had a higher risk of adverse events compared to FNA needles. The AASLD criterion was more difficult to achieve than the commonly-used criterion.

Endohepatology - current status

Jerome C. Edelson et al.

Summary: This article discusses the recent studies in the field of endohepatology, focusing on the use of diagnostic and therapeutic endoscopic tools for managing liver disease and portal hypertension. The findings indicate that different approaches for liver biopsy, including Endoscopic ultrasound (EUS)-guidance, percutaneous approach, and transjugular approach, are all clinically acceptable and have similar rates of adverse events. EUS-guided portal pressure measurement is safe, effective, and accurate. Combination therapy of EUS-guided cyanoacrylate injection and coil embolization is more effective and has fewer complications than individual therapy for gastric varices bleeding. Additionally, the use of artificial intelligence in conjunction with EUS shows high sensitivity and specificity in the diagnosis of malignant focal liver lesions.

Summary: This study compared the efficacy of a novel 22G Franseen-tip fine-needle biopsy device with 19G needle platforms for liver parenchyma biopsy. The results showed that the 22G Franseen-tip needle obtained more tissue fragments but had inferior specimen adequacy compared to 19G non-Tru-Cut needles.

Comparison of fine-needle aspiration and fine-needle biopsy devices for endoscopic ultrasound-guided sampling of solid lesions: a systemic review and meta-analysis

Priscilla A. van Riet et al.

Summary: The meta-analyses showed that fine-needle biopsy (FNB) outperformed fine-needle aspiration (FNA) in obtaining tissue samples, with higher diagnostic accuracy and tissue core rate, and requiring fewer passes for diagnosis. Among different FNB devices, forward-facing bevel needles demonstrated higher diagnostic accuracy compared to reverse bevel needles. However, due to low quality of evidence, further research is needed to make strong recommendations on the optimal FNB design.

Summary: The study found that in evaluating solid pancreatic lesions, EUS-FNB without ROSE can achieve high diagnostic accuracy. EUS-FNB without ROSE showed higher tissue core rate and shorter sampling time. Therefore, when using new-generation FNB needles, ROSE should not be routinely recommended.
