Global burden of bacterial antimicrobial resistance in 2019: a systematic analysis

Christopher J. L. Murray et al.

Summary: Antimicrobial resistance (AMR) poses a significant threat to global human health, and this study provides the most comprehensive estimates of AMR burden to date. By estimating deaths and disability-adjusted life-years (DALYs) associated with bacterial AMR in 204 countries and territories in 2019, the study highlights the impact of resistance and the leading pathogen-drug combinations contributing to it.

A national, multicentre, web-based point prevalence survey of antimicrobial use and quality indices among hospitalised paediatric patients across South Africa

P. P. Skosana et al.

Summary: Data on antimicrobial consumption among paediatric population in public hospitals in South Africa is limited. This study aimed to quantify antimicrobial usage, identify and classify which antimicrobials are used based on the WHO AWaRe list. The study found that antimicrobial prescribing is common among paediatric patients in South Africa and interventions should be targeted to improve prescribing practices and encourage greater use of oral antibiotics.

General Practitioners', Pharmacists' and Parents' Views on Antibiotic Use and Resistance in Malta: An Exploratory Qualitative Study

Hager Ali Saleh et al.

Summary: This study aimed to gain a better understanding of the antibiotic prescribing-use-dispensation dynamic in Malta from the perspective of GPs, pharmacists, and parents. The results showed that while there was generally high awareness of antibiotic overuse and ABR among the interviewees, issues such as antibiotic demand, non-compliance, and over-the-counter dispensing still existed. However, the interviewees believed that the public is more accepting of alternative strategies, such as delayed antibiotic prescription. Both GPs and pharmacists were enthusiastic about their roles as patient educators in raising knowledge and awareness in this context. Overall, there is room for improvement in antibiotic use in Malta.

Summary: Misconceptions and pressures have led to the increase in the illegal sales of antibiotics without prescription in Vietnam. The study found that a majority of antibiotic buyers did not have a prescription and many were unaware of the illegality of their actions. Occupation and education level were factors associated with purchasing antibiotics without a prescription. Recommendations include increasing fines, improving drugstore monitoring, promoting the family doctor system, and enhancing media and educational campaigns.

A cross-sectional study of antimicrobial use among self-medicating COVID-19 cases in Nyeri County, Kenya

George Kimathi et al.

Summary: During the COVID-19 pandemic, a high proportion of confirmed cases in Nyeri County, Kenya engaged in self-medication with antimicrobials, mainly antibiotics. The study found that older individuals were less likely to self-medicate, while those who experienced COVID-19 symptoms and had greater awareness of self-medication were more likely to engage in self-medication. Public health interventions should address the risks of irrational antimicrobial use during infectious disease outbreaks.

This exploratory study investigates the current status of vaccine uptake, challenges, and lessons learnt across 14 African countries. The study found variations in vaccine coverage, with low rates in some countries and increased hesitancy due to misinformation. Governments need to address vaccine hesitancy through targeted interventions among healthcare workers, journalists, and the public.

Antimicrobial Stewardship Strategies Including Point-of-Care Testing (POCT) for Pediatric Patients with Upper-Respiratory-Tract Infections in Primary Care: A Systematic Review of Economic Evaluations

Costanza Vicentini et al.

Summary: Upper-respiratory-tract infections (URTIs) are a common reason for antibiotic prescriptions in pediatric patients. However, many of these prescriptions are inappropriate as the majority of URTIs are caused by viral agents. This systematic review found that point-of-care tests (POCTs) could be a valuable tool for distinguishing bacterial from viral URTIs in primary care settings. Several studies concluded in favor of strategies that included POCTs, with some also finding them to be cost-effective.

Summary: This study aimed to analyze the impact of implementing clinical guidelines on the rate of antibiotic prescription in upper respiratory tract infection cases in an emergency department in Quito, Ecuador. The results showed that the implementation of clinical guidelines significantly reduced the rate of antibiotic prescription and increased appropriate antibiotic prescription rates, with the effects persisting over time. Early implementation of the guidelines showed more remarkable results compared to later stages. The emphasis in healthcare should shift towards strategy implementation rather than just guideline development.
