A Review of Current and Emerging Trends in Donor Graft-Quality Assessment Techniques

Summary: The number of patients on kidney transplant waiting lists is increasing, resulting in a gap between organ demand and availability. To address this shortage, medical professionals have started using marginal kidneys from expanded criteria donors (ECD). However, recipients of ECD kidneys tend to have worse outcomes compared to those receiving organs from standard criteria donors (SCD). Therefore, there is a need for representative methods to assess the quality of these transplant grafts. Current evaluation methods are limited and include interpreting laboratory tests, clinical risk scores, visual evaluation of the organ, and in some cases, a biopsy and perfusion parameters. New technologies have emerged in recent years to examine organ function and understand the mechanisms associated with complications. This review summarizes current diagnostic methods and new potential strategies for graft-quality assessment, identification of complications, perfusion control, and prediction of outcomes.

Summary: Subjects with high-normal urinary albumin levels, even without meeting the criteria for chronic kidney disease, already have an increased risk of cardiovascular events and kidney disease progression. Early intervention in this subclinical stage may reduce the risk. However, there are limited tools to predict albuminuria development and identify individuals who will benefit the most from intervention. Recent data have identified changes in urine molecules that reflect subclinical organ damage and processes such as inflammation.

Pretransplant kidney transcriptome captures intrinsic donor organ quality and predicts 24-month outcomes

Summary: With the development of omics technologies, transplant medicine is entering the era of precision medicine. This study aimed to identify predictive biomarkers for posttransplant kidney function using pretransplant gene expression profiles. Through analyzing a large number of kidney tissue samples, the study identified multiple genes associated with 24-month graft function. A gene model was developed to accurately predict the function of transplanted kidneys. These findings support the potential of pretransplant transcriptomic biomarkers as novel instruments for improving posttransplant outcome predictions and management.

Survival benefit of accepting kidneys from older donation after cardiac death donors

Summary: The study demonstrates that accepting kidneys from donors aged 50 or older after cardiac death can significantly decrease the mortality risk for recipients, with comparable survival benefits to other donor types. After declining DCD50 kidneys, some candidates still received kidney transplants from other sources at a lower survival rate.
