Paucigranulocytic Asthma: Potential Pathogenetic Mechanisms, Clinical Features and Therapeutic Management

Andriana Papaioannou et al.

Summary: Asthma is a heterogeneous disease with different inflammatory phenotypes. Identifying the specific phenotype is useful for selecting appropriate treatment. Paucigranulocytic phenotype of asthma (PGA) is a special subtype characterized by low levels of eosinophils and neutrophils in the sputum. The exact characteristics and pathobiology of PGA are not fully understood, but it may be driven by different cell types or represent a non-inflammatory phenotype with low levels of eosinophilic inflammation.

Molecular markers of type 2 airway inflammation are similar between eosinophilic severe asthma and eosinophilic COPD

Summary: Molecular markers of type 2 airway inflammation do not differ between eosinophilic asthma and eosinophilic COPD; however, the relationship between eosinophilia and type 2 airway markers appears weaker in COPD than in severe asthma.

Breathomics for the clinician: the use of volatile organic compounds in respiratory diseases

Summary: Exhaled breath analysis is promising for providing valuable insights into metabolic pathways in the lungs and other vital organs, but faces challenges such as lack of unified consensus for breath collection and complex relationships between exhaled VOCs and metabolic pathways. Further research and large-scale multicentre diagnostic studies are needed to advance breathomics in respiratory disease diagnostics.

The presence of neutrophils did not have a significant impact on asthma severity.

Clinical evaluation of type 2 disease status in a real-world population of difficult to manage asthma using historic electronic healthcare records of blood eosinophil counts

Adnan Azim et al.

Summary: Blood eosinophil measurement plays a central role in phenotypic characterization and guiding specific treatment approaches for difficult asthma patients. Persistent eosinophilia is associated with worse airway obstruction and higher FeNO levels, while patients who never show eosinophilia maintain better lung function and lower inflammatory markers. Longitudinal electronic healthcare record analysis provides insight into disease progression and can help guide personalized treatment strategies for difficult asthma.

Summary: Eosinophils are crucial in immune responses, combatting infections and diseases, while also playing pathological roles in various conditions. Biologic therapies targeting eosinophils have shown success, but further investigation is needed for patients who do not respond well. Research has revealed eosinophils' involvement in homeostatic processes, highlighting the need for more understanding in both disease and health contexts.

Baseline FeNO as a prognostic biomarker for subsequent severe asthma exacerbations in patients with uncontrolled, moderate-to-severe asthma receiving placebo in the LIBERTY ASTHMA QUEST study: a post-hoc analysis

Summary: In this study, higher baseline FeNO levels were associated with greater risk of severe asthma exacerbations in patients with uncontrolled, moderate-to-severe asthma, especially when combined with elevated eosinophil count and prior exacerbations, supporting the added value of FeNO as a prognostic biomarker. Further research is needed to confirm FeNO as an independent predictor for asthma exacerbations.

Recent Insights into the Management of Inflammation in Asthma

Hitasha Rupani et al.

Summary: The prevailing inflammatory paradigm in asthma is T2-high inflammation orchestrated by key inflammatory cells and associated cytokines. Various inflammatory biomarkers have been adopted in clinical practice to aid asthma characterization. Immunobiology of Steroid-Unresponsive Severe Asthma

Courtney Lynn Marshall et al.

Summary: Asthma is a heterogeneous respiratory disease characterized by airflow obstruction, bronchial hyperresponsiveness, and airway inflammation. Approximately 10% of asthma patients suffer from uncontrolled severe asthma (SA), which is resistant to common glucocorticoid treatments. Limited understanding of immune responses and molecular networks hinders the development of new therapies for SA.
