Extracorporeal Support Prognostication-Time to Move the Goal Posts?

Neel Shah et al.

Summary: Advances in ECMO technology have led to expanded indications and increased interest in developing prognostication tools to aid in bedside decision making. Current scores are limited to registry-based data with mortality as the main outcome of interest, highlighting the need for more clinically applicable decision support tools to improve long-term patient outcomes. More research is needed to address the gap in timing ECMO cannulation effectively.

Validation of Prognostic Scores in Extracorporeal Life Support: A Multi-Centric Retrospective Study

Christoph Fisser et al.

Summary: The study compared and validated multiple prediction scores in different ECMO patient populations, with SAPS II showing the best discrimination among survivors and non-survivors in V-A ECMO patients, while PRESET score performed the best in V-V ECMO patients. The prognostic performance of most scores was moderate in ECMO patients, discouraging their use in deciding ECMO implementation in potential candidates.

Early Findings after Implementation of Veno-Arteriovenous ECMO: A Multicenter European Experience

Aaron Blandino Ortiz et al.

Summary: This study reported the early effects of using veno-arterial-venous (V-AV) ECMO to improve hemodynamic and respiratory parameters in patients with ECMO. The findings showed that after V-AV implantation, there were significant improvements in heart rate, norepinephrine dose, PaO2, SaO2, and lactate levels. Additionally, there was a significant increase in the overall ECMO blood flow, distributed into the arterial and venous return cannulas.

Summary: The study examined the use of ECMO for COVID-19 patients during 2020 globally, showing lower in-hospital mortality rates in early-adopting ECMO centres and higher rates in late-adopting centres.

Conversion from Venovenous to Venoarterial Extracorporeal Membrane Oxygenation in Adults

Lars Falk et al.

Summary: The study found that patients converted from VV to VA ECMO had a higher mortality rate and lower prognosis scores, especially those converted due to right ventricular failure. VA ECMO is recommended as the first choice for patients with respiratory failure and signs of circulatory impairment. Furthermore, pre-admission RESP score, daily echocardiography, and SOFA score trajectories may help in early identification of patients needing conversion therapy.
