Proximal junctional kyphosis in pediatric spinal deformity surgery: a systematic review and critical analysis

Summary: This study conducted a systematic review and critical analysis on the risk factors of Proximal Junctional Kyphosis (PJK) in pediatric and adolescent spinal deformity surgery, with findings indicating that greater sagittal plane correction is a key risk factor for PJK in both EOS and AIS populations. Factors such as distractions, disruption of posterior elements, fusion constructs, and pre-operative spinal conditions were also found to contribute to PJK with Grade B evidence.

Biomechanical Analysis of the Reasonable Cervical Range of Motion to Prevent Non-Fusion Segmental Degeneration After Single-Level ACDF

Summary: This study aimed to determine the reasonable range of motion (ROM) without increasing facet joint force and intradiscal pressure (IDP) in non-fusion segments after anterior cervical discectomy and fusion (ACDF). The results showed that the compensatory increase in ROM in adjacent segments was more significant than that in non-adjacent segments, and the intervertebral ROM of all non-fusion levels increased in the ACDF model. By reducing the intervertebral ROM within reasonable ranges, the postoperative increase in facet joint force and intradiscal pressure in non-fusion segments can be offset.

Incidence of Proximal Junctional Kyphosis With Pedicle Screws at Upper Instrumented Vertebrae in Posterior Spinal Fusion for Adolescent Idiopathic Scoliosis

Summary: The study aimed to evaluate the impact of instrumentation type on the incidence of proximal junctional kyphosis (PJK) following posterior spinal fusion for adolescent idiopathic scoliosis. Results showed that PJK was significantly more common in all-pedicle screw constructs, with higher thoracic kyphosis before surgery. Using hooks at the upper instrumented vertebrae might be a potential treatment strategy to limit the occurrence of PJK.
