Analysis of risk factors for multidrug-resistant pathogens and appropriate treatment indications for pneumonia in children with neurologic impairment

Summary: The study found that children with neurologic impairment who develop pneumonia and receive tube feeding and/or have tracheostomy are more prone to multi-drug resistant (MDR) pathogens. Treatment success with broad-spectrum antibiotics was higher in these patients. Therefore, guidelines for treating pneumonia in children with neurologic impairment should take into account the risk factors for MDR pathogens.

There were 39 positive blood cultures, with 14 Gram positive and 18 Gram negative bacterial isolates, including 5 isolates resistant to empiric antibiotics.

Clinical factors associated with bloodstream infection at the emergency department

Summary: This study identified six clinical factors that are associated with positive blood culture upon arrival at the emergency department, including age over 55 years, moderate to severe chronic kidney disease, solid organ tumor, liver disease, history of chills, and body temperature over 38.3 degrees Celsius. These factors can be used as predictors of bloodstream infection.

Prospective cohort studies are needed to further explore potentially modifiable predictors of treatment failure.
