Comparison of Complement Pathway Activation in Autoimmune Glomerulonephritis

Dominique S. Genest et al.

Summary: In autoimmune glomerulonephritis, both the classical pathway and alternative pathway contribute to the activation of the terminal pathway. Therefore, both pathways may be potential therapeutic targets.

Complement induces podocyte pyroptosis in membranous nephropathy by mediating mitochondrial dysfunction

Hui Wang et al.

Summary: The study reveals the critical role of pyroptosis in podocyte damage mediated by complement activation in membranous nephropathy. It also uncovers mitochondrial dysfunction as an important mechanism underlying pyroptosis. These findings provide new insight and potential therapeutic targets for the treatment of membranous nephropathy.

Advances in the Management of Primary Membranous Nephropathy and Rituximab-Refractory Membranous Nephropathy

Maxime Teisseyre et al.

Summary: Primary membranous nephropathy (pMN) is an autoimmune disease characterized by damage to the glomerular basement membrane due to auto-antibodies. The conventional treatment approach for pMN involves uniform management, regardless of disease severity. However, recent advances have led to a more personalized management based on prognostic markers. Rituximab, an immunosuppressive drug, has shown to be effective in achieving remission in a majority of patients with pMN. However, there are cases of rituximab resistance, which pose challenges to treatment. Immunomonitoring may aid in overcoming treatment resistance by identifying possible causes and adjusting the treatment accordingly.

Identification of Hub Genes and Immune-Related Pathways for Membranous Nephropathy by Bioinformatics Analysis

Xiao-Yu Cai et al.

Summary: By utilizing bioinformatics analysis, this study identified 370 DEGs and 20 hub genes associated with membrane nephropathy, revealing a connection to cell damage and immune responses. The hub genes CTSS, ITGB2, and HCK were found to potentially play important roles in the pathological process of MN.

Summary: This study analyzed the serum of patients with THSD7A-associated membranous nephropathy and found that predominantly IgG4 anti-THSD7A antibodies can activate the complement system, but at high IgG4 density and through the alternative pathway.

The correlation of anti-β2-glycoprotein I antibody with complement activation in patients with idiopathic membranous nephropathy

Qiankun Zhang et al.

Summary: This study found that in patients with IMN, those with anti-beta 2-GPI antibodies had a significant decrease in the residual activity of the alternative complement pathway. There was no significant difference in the positive rate of C4d staining for patients with or without anti-beta 2-GPI antibodies in IMN patients, and the positive rate of anti-PLA2R antibodies was also the same.

Clinical significance of glomerular C3 deposition in primary membranous nephropathy

Ozgur Akin Oto et al.

Summary: This study found that the extent of glomerular C3 deposition is associated with the development of kidney failure in patients with primary membranous nephropathy. Patients with high intensity C3 deposition had lower eGFR, higher proteinuria, and were more likely to experience kidney failure compared to those with low intensity C3 deposition.

Rituximab in Membranous Nephropathy

Philipp Gauckler et al.

Summary: Membranous nephropathy is the most common cause of primary nephrotic syndrome among adults, and rituximab is now considered a first-line treatment option due to its favorable safety profile. However, questions remain about the optimal use of rituximab, and further research is needed to address these uncertainties.

Summary: The study indicates a close association between complement activation and the pathogenesis of membranous nephropathy (MN), with significantly higher levels of complement in patients with MN compared to normal controls, and lower levels of the complement regulator CR1. Levels of urine Ba, CSa, and MAC were also found to be correlated with the disease.

David R. W. Jayne et al.

Summary: In this study, the C5a receptor inhibitor avacopan was found to be noninferior to prednisone taper in achieving remission at week 26, but superior in achieving sustained remission at week 52 in patients with ANCA-associated vasculitis. All patients received cyclophosphamide or rituximab. The safety and clinical effects of avacopan beyond 52 weeks were not addressed in the trial.
