Effects of viremia and CD4 recovery on gut microbiome-immunity axis in treatment-naive HIV-1-infected patients undergoing antiretroviral therapy

Summary: Our study compared the differences in fecal microbial composition, serum and fecal microbial metabolites, and serum cytokine profile between treatment-naive patients before starting antiretroviral therapy (ART) and after achieving virological suppression after 24 weeks of ART. We found modest alterations in microbiota composition and changes in serum metabolites and cytokine levels in the viral suppression condition. In addition, we detected differences in microbiota composition and metabolites between immunological responders (IRs) and immunological non-responders (INRs).

Summary: The immune status of HIV-infected individuals is related to the microbial communities in the lungs. HIV-positive individuals have lower bacterial diversity in the lungs compared to HIV-negative individuals. There are significant differences in the composition and distribution of bacteria and fungi between the two groups. The number of fungal species in the BALF of HIV-positive patients is higher. The diversity of bacteria and fungi in the BALF of HIV-positive patients increases with decreasing CD4 T-cell counts.

Plasma MicroRNA Signature Panel Predicts the Immune Response After Antiretroviral Therapy in HIV-Infected Patients

Summary: Approximately 10-40% of HIV-infected individuals fail to achieve immune function improvement after ART, increasing the risk of non-AIDS-related diseases. This study identified five upregulated miRNAs in the plasma of immune non-responders, which were negatively correlated with CD4(+) T cell counts. A predictive model using these miRNAs accurately distinguished between immune non-responders and responders, with further mechanistic insights provided on the role of miR-16-5p in CD4(+) T cell function regulation.

Summary: Platelet-T cell aggregates play a critical role in maintaining inflammation in chronic HIV-1 infection. The formation of platelet-CD4(+) T cell aggregates was increased in treatment-naive HIV-1-infected individuals compared to healthy controls. Higher levels of these aggregates were associated with HIV-1 permissiveness and immune activation during HIV-1 infection.

Summary: HIV-infected immune non-responders (INR) exhibit higher proportions of CD4 T cell subsets expressing markers of exhaustion and senescence, as well as elevated plasma levels of IL-6, IP10, and sCD14. INR have higher levels of IP10 and lower levels of TGF-beta, with decreased cycling proportions of CD4 T regulatory cells. Proportions of T cells expressing TIGIT, PD-1, and CD57 positively correlate with IL-6 levels, while TGF-beta levels negatively correlate with proportions of TIGIT+ and PD-1+ T cell subsets.

Association between CD4+ T cell counts and gut microbiota and serum cytokines levels in HIV-infected immunological non-responders

Summary: Our study revealed that lower CD4(+) T cell counts in immunological non-responders (INRs) with HIV infection are associated with reduced abundance of Ruminococcaceae in the gut and elevated serum pro-inflammatory cytokines levels. This suggests that interventions targeting the gut microbiota to increase CD4(+) T cell counts could be a potential strategy for promoting immune reconstitution in HIV-infected INRs.

Summary: In this study, plasma metabolomic profiles of people with HIV (PWH) on antiretroviral therapy were evaluated to identify metabolite alterations associated with immune recovery. Results showed downregulation of indole-3-propionic acid, upregulation of cysteine pathways, and perturbations in serine and glycine metabolism and bile acid biosynthesis in PWH. These findings provide insights into the interplay of metabolism and immune response in HIV infection and offer potential biomarker candidates for immune recovery in PWH on antiretroviral therapy.
