Long-term mortality and cardiovascular events in patients with unilateral primary aldosteronism after targeted treatments

Summary: Long-term outcomes of unilateral primary aldosteronism (uPA) patients who underwent medical or surgery-targeted treatment showed that operated patients, especially those achieving hypertension remission, had lower risks of all-cause mortality compared to essential hypertension (EH) patients. Adrenalectomy was associated with lower mortality, MACE, and congestive heart failure risks compared to mineralocorticoid receptor antagonist (MRA) therapy in uPA patients.

International Histopathology Consensus for Unilateral Primary Aldosteronism

Summary: The HISTALDO consensus is useful to standardize nomenclature and achieve consistency among pathologists for the histopathologic diagnosis of unilateral PA. CYP11B2 immunohistochemistry should be incorporated into the routine clinical diagnostic workup to localize the likely source of aldosterone production.

Summary: In surgically treated patients with unilateral primary aldosteronism, there are differences in histopathology, genotype, and postsurgical outcomes between classical and nonclassical groups. Patients with nonclassical histopathology have a higher incidence of postsurgical disease persistence and increased aldosterone production from the unresected gland.
