Early Outcomes of Transconal Repair of Transseptal Anomalous Left Coronary Artery From Right Sinus

Summary: A novel trans-conal approach was utilized for 7 consecutive patients with anomalous aortic origin of the left coronary artery (AAOCLA) behind the right ventricular outflow tract (RVOT). The surgical repair involved transection of the RVOT, unroofing the septal course of the AAOLCA, and extending the RVOT with autologous pericardium. Postoperative evaluation showed anatomically patent unroofed AAOLCA with improved indexed fractional flow reserve. This technique showed excellent early outcomes, highlighting the importance of multimodality provocative testing in evaluating these lesions.

Summary: Anomalous aortic origin of a coronary artery (AAOCA) is a rare congenital disease associated with increased risk of myocardial ischemia, ventricular arrhythmias, and heart failure. This study presents a diagnostic approach for a symptomatic patient with right anomalous coronary artery and concomitant atherosclerotic disease, demonstrating that direct invasive dobutamine/volume FFR can assess the hemodynamic relevance of the anomalous segment after stenting the concomitant atherosclerotic stenosis. The patient was successfully managed without open-heart surgery for AAOCA based on this approach.
