Lung function in children with cystic fibrosis in the USA and UK: a comparative longitudinal analysis of national registry data

Daniela K. Schluter et al.

Summary: A previous analysis found that US pediatric CF patients had higher lung function compared to the UK, a difference that remained throughout childhood. There were no significant differences in early growth, nutrition, or infections, but differences in early treatments were observed.

Longitudinal changes and regional variation of incident infection rates at cystic fibrosis centers, United States 2010-2016

Marianne S. Muhlebach et al.

Summary: Changes in incident infection rates (IIR) for Pseudomonas aeruginosa (PA) and methicillin resistant Staphylococcus aureus (MRSA) were assessed across CF care centers, and factors such as location, ethnic background, and socio-economic variables were found to influence the rates. The IIR decreased for MRSA and PA at pediatric centers, while only PA IIR decreased at adult centers. Private insurance and a higher proportion of LatinX patients were associated with lower MRSA IIR, while larger center size, higher proportion of LatinX, and lower mean center-wide lung function were associated with higher PA IIR.

Human Multi-Compartment Airways-on-Chip Platform for Emulating Respiratory Airborne Transmission: From Nose to Pulmonary Acini

Eliram Nof et al.

Summary: Over the past decade, there have been significant efforts to develop novel preclinical in vitro lung models for pulmonary research. This includes the use of organ- and lung-on-chips, which have become particularly relevant in the study of respiratory viruses like SARS-CoV-2. In this study, the authors designed and fabricated a multi-compartment human airway-on-chip platform to investigate the crosstalk between different lung regions during viral infection. The platform successfully demonstrated viral transmission and inflammation pathways within the respiratory system, providing a valuable tool for preclinical pulmonary research.

Correction of CFTR function in intestinal organoids to guide treatment of cystic fibrosis

Anabela S. Ramalho et al.

Summary: In patients with CF, CFTR function in intestinal organoids correlates closely with sweat chloride values, and the rescue of CFTR function by CFTR modulators in organoids is tightly associated with improvements in lung function and sweat chloride levels reported in clinical trials. Candidate genotypes for modulator therapy have been identified based on organoid results, suggesting that organoids may serve as a potential in vitro model to guide precision medicine for patients with rare CFTR mutations.

Evolution of Pseudomonas aeruginosa toward higher fitness under standard laboratory conditions

Igor Grekov et al.

Summary: This study showed that hypermutator strains of Pseudomonas aeruginosa evolved towards higher fitness under planktonic conditions and perturbed the genetic context of membrane and cell wall biosynthesis genes. The complex regulatory circuits involving di-guanylate modulating enzymes played a key role in the transition between different lifestyles, allowing for increased growth performance across clonal lineages.

Summary: The Epidemiologic Study of Cystic Fibrosis (ESCF) was a large-scale observational study conducted on cystic fibrosis patients, collecting extensive data on disease characteristics, treatment outcomes, and survival rates. The study has facilitated the updating of CF patient genotype and survival information, providing valuable insights for patient care practices and future data registries.

Combining Ivacaftor and Intensive Antibiotics Achieves Limited Clearance of Cystic Fibrosis Infections

Samantha L. Durfey et al.

Summary: Studies show that chronic CF infections may persist despite improvements induced by CFTR modulators and aggressive antibiotic treatment. Combining CFTR modulators with intensive antibiotics may be most effective for patients with higher CFTR activity. Challenges remain in improving the health of people with CF.

From genotype to phenotype: adaptations of Pseudomonas aeruginosa to the cystic fibrosis environment

Laura Camus et al.

Summary: Pseudomonas aeruginosa is a main microbial species in the lungs of cystic fibrosis patients, capable of establishing chronic infections in hostile environments. Studies have identified convergent genetic mutations, transcriptomic, and proteomic dysregulations in adapted strains, leading to specific phenotypes that promote persistent infection.

A Phase 3 Open-Label Study of Elexacaftor/Tezacaftor/lvacaftor in Children 6 through 11 Years of Age with Cystic Fibrosis and at Least One F508del Allele

Edith T. Zemanick et al.

Summary: Elexacaftor/tezacaftor/ivacaftor (ELX/TEZ/IVA) has been shown to be safe and efficacious in children aged 6 through 11 with at least one F508del-CFTR allele, supporting its use in this patient population. The treatment improved lung function, respiratory scores, lung clearance, sweat chloride levels, and body mass index in these children over the 24-week period. The safety and pharmacokinetic profiles were consistent with those observed in older patients.

Take my breath away: studying pathogen invasion of the human lung using primary tissue models

Amanda L. Dragan et al.

Summary: The human pulmonary environment is complex, with a variety of cells that mount a strong response against pathogens, but some bacterial pathogens exploit this environment for replication and survival. Current lack of human disease-relevant model systems for studying these bacterial infections in the lung presents a challenge.

Transmission, adaptation and geographical spread of the Pseudomonas aeruginosa Liverpool epidemic strain

Matthew P. Moore et al.

Summary: The Liverpool epidemic strain (LES) is a significant transmissible clonal lineage of Pseudomonas aeruginosa that chronically infects cystic fibrosis patients' lungs. By sequencing 91 LES isolates from multiple CF centers in the UK, researchers identified divergence and adaptation in the spread of LES within the UK and between continents. Phylogenetic analysis revealed distinct clonal lineages among the 145 LES genomes analyzed, with clustering by clinic geography observed in the UK.

Summary: Human rhinovirus is a major risk factor for chronic obstructive pulmonary disease and asthma exacerbations. Research on host responses to RV infection in human lung tissue has shown similarities to gene expression changes in patients with COPD and asthma. The antiviral compound rupintrivir partially inhibits disease-relevant biological mechanisms.
